Applicant Acknowledgement of Drug Screening and Medical Exam
ABEC conducts its business with a high regard for safety in the public interest and in the interest of all its employees and toward maintaining the highest possible level of public trust. Because of this, ABEC has established a drug free workplace and asks applicants to undergo urinalysis screening for controlled substance or drug use.
I acknowledge notice of ABEC's Drug Screening Program and voluntarily consent to undergo a medical evaluation as ABEC may require, in accordance with the Americans with Disabilities Act. I understand that my medical examination will include a urinalysis and that, in addition to routine testing, my urine specimen will be screened for drugs including, but not limited, to Amphetamines, Barbiturates, Benzodiazepines, Cocaine, Marijuana, Methadone, Methaqualone, Opiates, Phencyclidine, and Propoxyphene.
I understand an unacceptable result in such testing will lead to my rejection from further employment consideration by ABEC. If I wish to appeal or contest a positive drug screen test, I must direct a written request to the Human Resources Generalist within 30 days of notification of my test results.
I understand that I have the right to refuse to take a drug screening test; however, my refusal will be considered by ABEC as my voluntary withdrawal.
Should I be tested, I will provide a listing of all drugs which I have used during the prior thirty days to the testing facility.
