ATTENTION ALL APPLICANTS

You are advised that completion of the application and interview process, if you receive an offer of employment, that following are conditions of employment:

  1. Current license or certification must be submitted, if applicable.
  2. Documents establishing identity and employment eligibility must be submitted upon completion of the Employment Eligibility Verification form I-9.
  3. Upon hire, all employees are required to complete Florida Department of Law Enforcement and Florida Agency for Health Care Administration Systems Level I Background Screening forms and receive clearance regarding abuse/neglect record prior to active employment, as required by regulatory agencies. (Employees who have not maintained continuous residency within the State of Florida for the last 5 years immediately preceding the date of request for background screening must complete a Level II screening, which includes a full set of fingerprints.) Effective January 1, 2005, every employee is required a Level II background check regardless of the number of years as a Florida resident.
  4. Due to regulatory agency requirements, employees must provide current documentation of freedom from communicable disease, or a post job offer medical examination is required. If a positive PPD skin test necessitates and x-ray, it must be secured at the employee’s expense. The information obtained in the course of an examination or submitted documentation is treated as a confidential medical record and maintained in a separate medical file. During Flu Season (October – March) ALL EMPLOYEES must obtain a mandated Flu Vaccine or provide documentation that Flu Shot has been received prior to employment.
  5. Employees shall be required to submit to any job related testing or examination, as required.
  6. Pre-employment drug testing is mandatory. Anyone refusing to submit to drug testing, refusing to sign the consent form, failing to appear for testing, tampering with the test results or failing to pass the pre-employment drug test will be ineligible for hire.
  7. Satisfactory references must be received from past employers (Complete information regarding dates and addresses are essential for processing.)
  8. All employees are required to serve an initial ninety days probationary period. The probationary period is an intrinsic part and extension of the employee selection process during which the employee will be considered in the training and under careful observation and evaluation by supervisory personnel. Employees may be terminated at any time without notice.
  9. Payroll checks are automatically deposited in a checking or savings account at your own financial institution through Direct Deposit. A voided check or savings account deposit form will be required to complete the new hire paperwork procedures.
