• Personal Statement of Faith/Personal Spiritual Pilgrimage - Write a paragraph about your spiritual journey relating to how you have developed in your knowledge and practice of the Christian faith.
• Membership in a Christian Community of Faith - Describe the considerations that drew you to your community of faith and the ways in which you serve in that community. Please include your Church Name, Name of Pastor, Church Address and Phone Number. Please explain in detail, if you do not have a church home and why. Describe what your intentions are for incorporating this vital spiritual practice into your Christian walk. (Not having a church home does not disqualify you from employment.)
• One (1) Letter of Reference from Current Pastor - Please attach a letter of reference from your current Pastor, an elder, deacon, or other church leader who can speak into your spiritual walk. Please explain in detail, if you do not have a current Pastor/Clergy representative.
• Two (2) professional letters of recommendation/references - Please provide two professional letters of recommendations/references.
