Who we are

The Vancouver Housing Authority (VHA) works closely with local governments and other community partners throughout Clark County to address issues of affordable housing and homelessness and help families break the cycle of poverty.


Who we are looking for

The VHA is seeking a Community Wellness Supervisor to supervise and support the day-to-day functions of the Community Wellness Specialists and provide them with adequate coaching, reflective supervision and support to ensure the quality of their work. The Community Wellness Supervisor will work collaboratively with the Supportive Services Manager regarding program management and operations. The Community Wellness Supervisor uses specific care coordination technology to monitor and approve Community Wellness Specialists’ documentation via community health record. The Community Wellness Supervisor will attend all SWACH meetings and trainings, and carries a small additional caseload.


What you'll do

  • Manages the day-to-day operations of the program.
  • Provides and/or manages the team to ensure compliance with facility, government, and contractual requirements.
  • Attends meeting with community groups, the Pathways HUB and with other VHA departments.
  • Maintains all appropriate releases of information, documentation of services provided, and other protected client information.
  • Carries a small case load to help support the team.
  • Conduct standardized in-take and risk assessment.
  • Work cooperatively with other Voucher Specialists in the Voucher Department.
  • Work collaboratively with the Supervisor to make sure that all client and program needs are identified and addressed.
  • Maintain confidentiality in regards to all client information.
  • Collect data for evaluation and quality improvement process.
  • Educates other staff on the issues related to area of specialty to help them to better work with and assist the residents.
  • Consistently complies with position and department safety and attendance standards.
  • Establishes and maintains positive, productive relationships with employees, residents, vendors, and other work related contacts.


What we offer

  • Starting pay rate is $4,064.00 to $4,267.00 per month based on experience.
  • Full-time, non-exempt position working 40 hours per week, Monday - Friday.

Requirements

What you'll bring

  • Bachelor’s degree in Social Services or related field.
  • Three years of care coordination or case management experience.
  • A minimum of one year's experience in a supervisor role preferred.
  • Minimum of two years’ experience in health education, chronic disease management, and/or housing navigation.
  • Experience working with low income, multi-ethnic populations preferred.
  • Bilingual preferred.
  • Knowledge of community resources in Clark County a plus.
  • Valid driver’s license.
