Description

With the integration of ResolutionCare Network services, we are actively looking for an individual with experience in social work in the healthcare industry. This position is part of an Interdisciplinary Team (IDT) under the supervision of the Director of Social Support Services. The individual in this role provides essential support and care to palliative care patients and families in collaboration with the RN Case Manager (CM) and IDT and assists the CM and the IDT in developing and implementing an effective plan of care for RC patients in support of the physical, psychological, and spiritual care of RC patients.

Requirements

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides medical social work case management to a given group of patients and their families
  • Provides social work services to patients and their families through video technology, and telephone calls
  • Assesses and responds to the psychosocial, practical, financial, and legal concerns of the patients and families/support systems
  • Assists patients and families in accessing and using community resources
  • Assists patients and families with the navigation of social and healthcare systems
  • Mediates conflicts between families and care networks
  • Completes financial needs assessments and assists with navigation on MediCare, MediCal, Disability and Social Security and Veterans Affairs benefits.
  • Assesses for spiritual and medical needs and communicates these promptly to interdisciplinary team
  • Establishes and maintains relationships with community resources
  • Supports and supervises social work interns
  • Identifies problems, goals, interventions and frequency of visits to complete the social work Plan of Care
  • Consults and collaborates with interdisciplinary team and others involved in the patient’s care on an ongoing basis updating the care plan as appropriate
  • Facilitates advance care planning, including advance directive and POLST completion.
  • Provides resources and assistance for funeral arrangements
  • Completes documentation within 48 hours
  • Assumes other duties in support of ResolutionCare’s needs as directed by the Clinical Director or Medical Director
  • Assists with referrals and enrolls patients as needed
  • Assists with community outreach and education regarding RC services


ORGANIZATIONAL REQUIREMENTS

  • Works at all times within the policies and guidelines of the company
  • Willingness to use video conferencing as frequently as appropriate
  • Ensures effective use of resources
  • Plays an active part in the development and maintenance of good relationships with all who have business with the Company
  • Respects at all times the confidentiality of information covering patients, staff and volunteers and is HIPAA compliant
  • Promotes the company’s philosophy of compassionate communication towards all patients, relatives, visitors and coworkers


ENVIRONMENTAL CONDITIONS AND PHYSICAL REQUIREMENTS

  • Works with Palliative Care patients remotely wherever they reside or are receiving care, including homes, skilled nursing facilities, hospitals and residential care for the elderly facilities
  • Successfully performs the essential duties and responsibilities of the position through the use of physical activities as described below
  • Regularly requires working at a computer for 2-4 hours at a time
  • Regularly requires use of telephone
  • Requires use of video-conferencing technology


QUALIFICATIONS AND EXPERIENCE

Minimum

  • MSW in Social Work or BSW with equivalent experience
  • Excellent oral and written communication skills required
  • Positive interpersonal skills required
  • Must have general computer skills and a working knowledge of MS Office and the internet


Preferred

  • Minimum 2 years health care social work experience
  • Experience in hospice, palliative care, oncology or home health
  • Palliative care certification
