Full-time: 35 hours, M-F, 8 a.m. – 4 p.m., 7 hours a day, 5 days a week (Schedule may vary)

Hourly rate: $18.50 - $20.00 per hour, depending on experience


The Chula Vista Community Collaborative (CVCC) works to ensure collaboration among partners and stakeholders in Chula Vista. CVCC draws together all sectors of the local community to develop coordinated strategies and systems that protect the health, safety, and wellness of residents. The role of CVCC is to work with partner organizations in Chula Vista and the South Bay to share information and resources that strengthen families and communities.  


Responsibilities:

Under the direct supervision of the CVCC Program Supervisor, the CVCC Community Health Worker / Promotor(a) will work out of five of the different Family Resource Centers (FRC’s) that make up Chula Vista Community Collaborative which is located in San Diego’s South Bay area. The CVCC Community Health Worker / Promotor(a) is responsible for conducting prevention activities which include in-services, community presentations, participation at health/resource fairs, and outreach & material dissemination to create awareness and refer families and caregivers to various programs available at each FRC. 


Duties:

  • Engage in collaboration with Chula Vista Elementary School District (CVESD) and Sweetwater Union High School District (SUHSD)
  • Provide education and coaching on various topics for different populations.
  • Establish and strengthen community partnerships and referrals for housing, childcare, food banks, employment, education counseling, legal services, attendance and other related services.
  • Prepare and execute educational presentations at schools and other community organizations
  • Conduct ongoing or telephonic and/or in person outreach visits with clients as needed. 
  • Create and implement written work (such as plans, flyers, reports, etc.) in a timely manner and with accurate data 
  • Attend weekly meetings as required virtually and/or in person (partnership, organizational and supervisory meetings) 
  • Serve as a representative for CVCC at local meetings or community events
  • Excellent verbal and written communication
  • Knowledge and experience working with minority populations.  
  • Possess and maintain positive and supportive relationships with coworkers 
  • Be able to maintain accurate knowledge about the agency, its programs and service.  
  • Accurately represent the agency to and maintain positive working relationships with the public, clients, funding sources, government bodies, etc.  
  • Communicate positive and negative feedback from the community through your supervisor to management.  
  • Utilize appropriate opportunities to pursue positive public relations for the agency.
  • Personal experience living or working in South Bay community to be served, preferred   
Requirements
  • Must have a High School Diploma or GED equivalent
  • Must be bilingual (English and Spanish) 
  • Must have awareness of available resources, community conditions, barriers, and assets.  
  • Must have experience working in a community-based organization 
  • Must take direction and work independently to execute weekly, monthly and quarterly goals 
  • Must be culturally inclusive and a leader in diversity 
  • Must be computer literate, including but not limited to Microsoft office Software and other database programs
  • Candidate is required to have reliable transportation; therefore the candidate must successfully complete a motor vehicle history check. Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy. Mileage reimbursement provided.

  • Lifting: Raising or lowering an object from one level to another (includes upward pulling) 25-50 lbs.
  • Carrying: Transporting an object, usually holding it in the hands or arms, or on the shoulder
  • Pushing: Exerting force upon an object so that the object moves away from the force.
  • Pulling: Exerting force upon an object so that the object moves toward the force (includes jerking)
  • Climbing: Ascending or descending stairs, using feet and legs or hands and arms. Body agility is emphasized
  • Balancing: Maintaining body equilibrium to prevent falling when walking, standing, crouching.
  • Stooping: Bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.
  • Handling: Seizing, holding, grasping, turning, or otherwise working with hand or hands. Fingers are involved only to the extent that they are an extension of the hand.
  • Finger Movement: Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling
  • Talking: Expressing or exchanging ideas by means of the spoken word. Talking is important for those activities in which workers must impart oral information to clients or to the public, and in those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly.
