The LPN and RN position provides direct nursing care to our residents, and supervises the day-to-day nursing activities preformed by nursing assistants ensuring that the highest degree of quality care is maintained in accordance with the resident’s care plan. 

Part time availability, AM, PM, NOC shifts available at both of our Sheboygan Falls locations. Great base pay rate, FTE premiums, shift differentials and a sign on bonus for part and full time. 

Essential Functions/Responsibilities:  

  • Direct the day-today functions of the nursing assistant
  • Receive physician's orders and execute as appropriate
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to each resident in EMR
  • Ensure appropriate documentation and communication of changes in a resident's needs and/or condition
  • Prepare and administer medications as ordered by the physician
  • Ensure that an adequate supply of supplies and equipment is on hand to meet the nursing needs of the residents
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician
  • Consult with the resident's physician in providing the resident's care and treatment
  • Review the resident's EMR for specific treatments, medication orders, diets, etc.
  • Maintain established nursing standards
  • Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status
  • Cooperate with and coordinate social and activity programs
  • Notify the resident's physician and P.O.A./Guardian of changes in condition
  • Carry out restorative and rehabilitation programs, to include self-help and care
  • Administer professional services such as; catheterization, tube feedings, wound care, etc.
  • Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes
  • Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is maintained at all times
  • Attend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.


  • Possess a current, active license to practice as an LPN or RN in the State of WI

  • Regular attendance, including timely arrival at work, to support positive working relationships with fellow team members and to support quality resident care
  • Ability to work in a fast paced rapidly changing environment

  • Ability to lift up to 40 pounds
Salary Description
$25.48 -$35.12 based on license and experience
