• Work directly with consumers, following their Day Habilitation Plan which is aimed toward assisting the consumers in developing competence in functioning as independently as possible.
• To relate to the consumers in a manner that ensures their rights to privacy, personal choice, and to a life of dignity and purpose.
• To follow dietary, medical or behavioral plans for each individual.
• Support consumers in personal hygiene, toileting, showering/bathing, dressing, grooming, feeding, etc. as determined by each one’s ability.
• To complete recording requirements such as data for valued outcomes as identified in the consumer’s Day Habilitation plan, incident/accident reports, daily communications logs, baseline data, and reports for habilitation meetings as assigned.
• Implement daily schedules, including all routines and life skills activities, and community inclusion, as directed by supervisor or written in the plan.
• Ability to encourage self-advocacy or to advocate as needed for the consumers.
• Ensure client health and safety, while assisting consumers to achieve increased independence.
• Perform job-related tasks as assigned, to include attendance and participation at meetings and training as necessary.
Salary up to $16.50 per hour and benefits after 30 days.
One year experience working with OPWDD population is preferred, potential to be certified in Driver Safety, SCIP-R, CPR/First Aid & AMAP
High School Diploma/GED Must have a Valid/Unrestricted N.Y.S Driver's License.
