Summary/Objective
The Corporate Health Coordinator leads the employer wellness program. The Coordinator collects information and acts upon employer health needs through screenings, coaching, and collaborating to understand employer goals. The Coordinator identifies employers’ unique population health needs and works closely with the employer to improve the employers’ overall population health. Essential Functions
- Demonstrates service excellence at all times. Exhibits excellent guest relations to patients, visitors, and team members.
- Works with a diverse population to assist in the development of individualized health plans of care/goals with other health and wellness staff members with a focus on small, steady, sustainable improvements for each patient or group.
- Facilitates, as needed, and reviews biometric screenings, Health Risk Assessments (HRAs), and additional information to understand priorities and goals by individual patients and/or groups.
- Participates in outpatient educational/activity efforts in small groups, large classes and/or individual sessions via in-person, web-based, or over the phone.
- Assists the patient with identifying additional resources or referrals needed and navigates the process to meet patient needs.
- Develops and continually improves group and individual therapy framework, providing tools and resources helpful to patients in meeting goals surrounding the ACLM lifestyle medicine framework.
- Contributes expertise as a member of a multidisciplinary team to offer comprehensive health and wellness strategies to provide positive outcomes for individual participants in employer and community programs.
- Demonstrates ability to communicate effectively with health team members and about program improvement issues.
- Collaborates with employer partners and patients to establish a culture of health through ongoing participation in meetings and projects and providing client-based programming outreach.
- Demonstrates interpersonal and observational skills needed to effectively identify appropriate psychosocial adaptation to illness and/or the presence of psychopathology (e.g., depression, anger, etc.).
- Understands and utilizes contemporary theories of health behavior change in helping participants identify learning and lifestyle modification goals.
- Effectively applies knowledge of health and exercise science to an identified population. Demonstrates teaching ability in large groups and one-on-one coaching sessions.
- Develops and adjusts individual health/exercise prescription based on feedback from physician/provider recommendations on a regular basis. Supervises exercise sessions in small groups, monitoring participants for inappropriate responses to exercise.
- Assess patients pre, during and post exercise making appropriate referrals and/or recommendations to participant and other staff.
- Documents initial and current assessment data in patients’ records, as appropriate.
- Participates in the collection, analysis and reporting of identified individual patient and program outcomes.
- Maintains standard precautions and applies principles related to patient care.
- Participates in ongoing education and professional development for department and self.
- Participates in continuous quality improvement activities (CQI).
- Participates in staff meetings and other meetings as required.
- Participates in health fairs and screening events.
Requirements
Qualifications
Education Required
- Graduate of an academic program with bachelor’s degree in healthcare-related field required, exercise science preferred. Certification from an ICF-accredited or NCCA coaching program (Wellcoaches, ACE, etc.) or ability to complete within first year of employment
- Bachelor’s Degree required; Master’s Degree preferred in healthcare related field.
Experience
- Experience and knowledge of The American College of Lifestyle Medicine (ACLM) and six pillars, American College of Sports Medicine (ACSM), or similar health and wellness coaching platform, preferred.
- Familiarity with EPIC, Microsoft Excel, PowerPoint and social media outlets is preferred.
- Experience with training programs for running, triathlons, and other group activity events is preferred.
Skills/Competencies
- This position requires professional communication and interaction with the public, co-workers, administration, and contacts from other hospitals, schools, business partners, and/or other departments.
- Area specific required education may be obtained upon hire and is to be maintained, e.g., BLS,
- Additional certifications in health/fitness specialties is preferred- Cardio, Strength, Yoga/Flexibility, etc.
