About Our Team
VyncaCare, is a fast-paced, entrepreneurial environment incredibly passionate about transforming end-of-life care. This individual works as part of an interdisciplinary team and is responsible for managing and coordinating patient care, including oversight of the care plan and provision of direct patient care. Care is provided in collaboration with other Care Team members as described in the patient’s care plan.
Essential Job Duties/ Responsibilities
The following is a representation of the major duties and responsibilities of this position:
- Will provide nursing services in accordance with a plan of care as ordered by the patient’s provider. Observes and evaluates treatment effect and patient's overt physical/emotional response to treatment goals.
- Plans goals and care that are reasonable, necessary and specific for the skills of the nurse. Regularly participates in interdisciplinary planning, case conferences and staff meetings as required.
- Observes, records and reports information on the patient's condition to the attending provider and interdisciplinary team, and in the patient's medical record.
- Documents patient assessment, treatment and plan accurately using established guidelines.
- Engages in case management and care coordination activities to support patients' plans of care.
- Manages the patients assigned to their Care Team, ensuring that visit frequencies by the RN and Interdisciplinary team are appropriate to the patient’s current acuity level and needs.
- Provides education to patients and caregivers regarding VyncaCare’s services and palliative care, including provision of educational materials about specific disease processes and symptom management, as well as caregiver teaching and the importance of medication management.

Environmental Conditions and Physical Requirements
- Provides palliative care to patients primarily in the home setting, with possible virtual work.
- Occasionally requires reaching, stooping, bending, kneeling, and lifting items weighing 50 pounds or less.
- Potentially performs procedures or other tasks that could involve exposure to blood, body fluids, or other potentially infectious materials.
- Regularly requires use of telephone and laptop.
Requirements
Minimum Qualifications:
- Current California Registered Nurse license
- Minimum 1 year of nursing experience in a clinical setting- preferably in palliative care, hospice, or home health.
- Minimum of one year of experience as a professional nurse within the previous three years
- Excellent judgment and knowledge of current nursing practices, with demonstrated exemplary clinical, analytical and documentation skills with little supervisory direction.
- Excellent oral and written communication skills required
- An active, licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
- Computer literate and willing to embrace technology used by the organization, including EMR, Google Suite, and other technology that supports the patient management and communication.
Preferred Qualifications:
- BSN
- CHPN certification or equivalent
