Position Summary:


We are seeking a dedicated and Compassionate RN to join our team on a full-time basis. This role offers a unique opportunity to work in the Northern area (Fort Kent) 3x/week and Central area (Caribou) 2x/week. Using the nursing process, the skilled RN will evaluate client's nursing needs, administer skilled nursing care to clients requiring professional nursing service and teaches and supervises the family and other members of the nursing team. 


Essential Functions:  

  • Conduct the admission and ongoing treatment of clients by obtaining medical history, physical examination, evaluation of home and environment, and other nursing related assessments as it pertains to the present condition(s).
  • Will initiate preventative and rehabilitative nursing procedures and coordinate specialized required services with other health professionals through the main office.
  • Initiates, evaluates and adjusts plan of care as needs of clients change.
  • Initiates actions and activities to ensure continuity of effective patient care before, during, and after the period of home care services.
  • Keeps physician, supervisors, client and/or responsible party informed of condition changes or needs.
  • Counseling and teaching client and/or primary care giver is expected to meet needs and learn self care techniques.
  • Contacts appropriate community agencies and other disciplines necessary to ensure continuity of care.
  • Supervise ancillary home health personnel and participates in their performance appraisals.
  • Maintain records and documentation as per agency's policies.
  • Participate in quality assurance, meetings, and in-service education as outlined in policies.
  • Work within the scope of policies and procedures established by the organization as well as other professional and regulatory standards. 





Requirements

Education, Skills and Experience Required:

  • Graduate of an approved nursing program with a valid Maine State license in good standing. Current BLS certification.
  • Has a minimum of one year experience in home health care or medical - surgical nursing.
  • Is physically and mentally healthy and possesses adequate communications skills to express self and write reports.
  • Has ability to relate well with the public, colleagues and other professional agencies.
  • Able to work alone in home situations, use good judgements and demonstrate decision making skills in critical or emergency situations.
  • Has ability to direct and/or supervise ancillary personnel.
  • Must have reliable transportation and be willing to travel to client's homes on assignments. Vehicle must be insured as per policy.
  • Availability to work at odd hours, week-ends and take call as assigned or as needed.

Will assume other responsibilities deemed necessary and in the best interest of the organization as requested by the Executive Director.
