Responsibilities:
As a Bereavement Coordinator at Snowline Hospice, you will provide compassionate and comprehensive support to patients’ families and the community. Your responsibilities will include:
- Assist the Bereavement Services Manager in planning, designing, implementing, facilitating, and managing bereavement programs, support groups, and related activities
- Ensure timely initial bereavement contact with hospice family survivors
- Coordinate, plan, facilitate, and participate in the Healing Altogether (HAT) program
- Provide grief education and support through letters, educational materials, individual outreach, support groups, and counseling sessions
- Respond to inquiries from community members regarding grief support, resources, and referrals
- Facilitate grief support groups and memorial events, including occasional evenings and weekends
- Collaborate with the interdisciplinary team to ensure continuity of care and compliance with hospice regulations
- Maintain accurate and timely documentation in applicable systems
Your Experience and Education:
- Master’s degree in social work or a counseling-related field from an accredited graduate school
- At least two (2) years of postgraduate experience, preferably in healthcare, hospice, or grief support
- Excellent communication, interpersonal, and counseling skills
- Demonstrated ability to support individuals and families experiencing loss and grief
- Experience designing and facilitating small groups
- Valid California driver’s license required; reliable insured vehicle or willingness to participate in the company car program
- Proficiency with computers and documentation systems required
- Prior electronic medical record (EMR) experience preferred
- Ability to work occasional evenings and weekends as needed
- Commitment to providing compassionate, patient-centered care
- Knowledge of trauma-informed care principles preferred
