Job Description:
We're seeking a passionate and dedicated Telehealth Care Coordinator to join our team. As a Telehealth Care Coordinator, your role will be essential in ensuring the effective coordination of patient care, streamlining communication between patients and healthcare professionals, and enhancing the overall patient experience.
Key Responsibilities:
- Schedule, coordinate, and manage virtual appointments between patients and healthcare providers.
- Assist patients in navigating our telehealth platform, including setting up accounts, troubleshooting technical issues, and answering inquiries about our services.
- Work closely with healthcare providers to facilitate the provision of telehealth services, including preparing and managing patient files, documentation, and follow-up care.
- Educate patients on the scope of services, processes, and the benefits of telehealth.
- Collaborate with multidisciplinary teams to develop care plans and manage patient cases.
- Follow up with patients to track progress, compliance, satisfaction, and gather feedback for continuous improvement.
- Maintain patient confidentiality and adhere to HIPAA guidelines and company policies.
- Stay current with advancements in telehealth and recommend enhancements to improve patient care and satisfaction.
Requirements
- Associate or bachelor's degree in healthcare administration, nursing, social work, or related field preferred.
- Prior experience in healthcare coordination, telemedicine, or patient support roles would be an advantage.
- Familiarity with healthcare software, Electronic Health Records (EHR) systems, and telecommunication technology.
- Exceptional interpersonal, verbal, and written communication skills.
- Strong organizational skills with the ability to multitask, prioritize, and manage time efficiently.
- Compassionate and patient-oriented with a strong commitment to providing exceptional patient care.
- Ability to work independently and collaboratively within a team environment.
