Esperanza Health Centers, Federally Qualified Health Center, is seeking a Nurse Practitioner, specializing in Family Medicine,  who is mission driven, passionate about healthcare and can provide innovative ideas for better health in the southwest Chicago communities. Our Nurse Practitioner provide primary care services to pediatric, adult and ob/gyn patients. The Nurse Practitioner primarily focus on emphasizing prevention of illness, promotion of healthy lifestyles while using primary care concepts.  


Primary Duties and Responsibilities:

  1. Manages ambulatory Family Medicine primary care medical practice by eliciting patient histories, performing physical examinations, providing accurate clinical diagnoses, prescribing evidence-based and cost-effective treatments and providing patient education.
  2. Adheres to Esperanza Health Center’s Productivity Standard for Nurse Practitioners/Advanced Practice Providers of an average of at least 2.3 patient visits per hour of ambulatory time worked.
  3. Participates in Quality Improvement activities of the health center including complete, legible and accurate chart documentation, Peer Review chart audits and reviews, and designated projects.
  4. Makes appropriate diagnoses and prescribes appropriate prescriptions and referrals consistent with Esperanza Health Center’s medical practice guidelines.
  5. Prepares, implements and documents age specific preventative health schedules for pediatric and adult – and in some cases OB/GYN – patients.
  6. Builds and maintains a Family Medicine medical practice with follow-up of return appointments and referrals, in-patient management and other methods that assure continuity of care.
  7. Maintains clinical competency through regular Continuing Medical Education and provides documentation of such to Esperanza Health Center’s Administration.
  8. Maintains liaison with any Partner Hospitals’ medical staff and ambulatory care departments through obtaining and maintaining Hospital Privileges, attendance at department and section meetings, and attending physician teaching duties
  9. Plays a role in educating and training medical and healthcare professionals, as needed, and accepts and implements teaching assignments of Health Center staff as assigned by the Medical Director.

Requirements
  1. Graduate of an Accredited School
  2. Current IL State License, Controlled Substance License, and DEA Registration
  3. Two or more years of experience or graduate of fellowship/residency program
  4. Bilingual English/Spanish, required
