Position Summary: 

We are seeking a dedicated and Compassionate RN to join our team on a per-diem basis. This role offers a unique opportunity to work in the Caribou and surrounding area. The Long-Term Care RN provides monthly assessment and supervision of care to clients of Aroostook Home Health Services. 


Essential Functions: 

I. Conduct a general health assessment - conduct direct/indirect visits. 

  • Conduct assessment and review and update the plan of care to accommodate client's needs. 
  • Check medications and review side effects. 
  • Fill medication boxes  
  • Administer flu shots, allergy shots and other injections if necessary. 
  • Perform venipuncture per physician's order, and collection of lab specimens as ordered. 
  • Trim fingernails and toenails if client or primary caregiver is unable to do so. 
  1. Conduct visits to assess quality of care provided to clients. 
  • Discuss with staff any problems involving the client. 
  • Admit clients as directed. 
  • Complete 485's as required. 

2.  Monitor compliance with AHHS' safety policies and procedures. 

  • Verify that Risk Assessment Profile addresses all safety issues. 
  • Verify the necessary safety equipment is available and being used appropriately. 
  • Update the Risk Assessment Profile and address new issues with staff per agency policy.


3.   Monitor compliance with all AHHS policies and procedures, licensing regulations and funding source requirements. 

  • Ensure staff complies with Plans of Care. 
  • Ensure Plan of Care complies with service authorizations. 
  • Enforce policies and procedures with staff/clients. 

4.   Carry out/ maintain compliance with AHHS policies and procedures. 

  • Follow universal precautions when providing client care 
  • Complete required and appropriate records and reports 
  • Maintain confidentiality 
  • Utilize the Electronic Visit Verification to log client visits and completed tasks

Location:

Requirements
  • Graduate of an approved nursing program with a valid Maine State license in good standing.
  • Has ability to relate well with the public, colleagues and other professional agencies.
  • Able to work alone in home situations, use good judgements and demonstrate decision making skills in critical or emergency situations.
  • Must have reliable transportation and be willing to travel to client's homes on assignments. Vehicle must be insured as per policy.
