Description

The Cottages of Dakota is seeking a dedicated Licensed Practical Nurse (LPN) to provide part-time nursing services for our Burnsville and Apple Valley sites. The ideal candidate would have flexible availability Monday thru Friday during business hours, as well as evening and weekend hours as needed. Our facilities cater to 27-30 individuals who require a minimal level of nursing oversight in a comfortable residential care setting.


Key Responsibilities:

  • Conduct daily rounds to monitor patient conditions and ensure continuity of care between the patient, house management, and staff.
  • Perform assessments, chart observations, and track the progress of each patient’s condition during each visit.
  • Train and ensure the competency of new staff members and continued education for current staff members  
  • Administer medications as prescribed by physicians and monitor vital signs.
  • Manage wound care, including dressing and assessing the healing process.
  • Educate patients and their families on proper home health care techniques and strategies to aid recovery.
  • Collaborate closely with physicians, physical therapists, occupational therapists, and other professionals involved in the patient’s care plan.
  • Provide palliative care to enhance patient comfort.
  • Recommend tools and devices that may improve the patient's quality of life.

Requirements

 Qualifications 

  • Degree from an accredited school of nursing and be licensed as an RN or LPN in the state of MN
  • Must be able to successfully pass a criminal background study with The Department of Human Services
  • Experience in Home and Community Based Services preferred, but not required
  • Resident centered thinking with awareness and sensitivity to resident needs
  • Committed to interacting professionally and with care toward residents, families and other staff
  • Ability to communicate effectively in both verbal and written form
  • Proven experience in a nursing role
  • Strong organizational and communication skills.
  • Ability to work flexibly on an on-call basis.
  • Capability to work independently and as part of a team.

  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

  • Must meet the general health requirements set forth by the policies of this program, which include a medical and physical examination.
  • Must be able to relate to and work with the disabled, emotionally upset, and, at times, hostile individuals in the program.
  • Must be able to push, pull, move, and/or carry a minimum of eighty (80) pounds a minimum distance of ten (10) feet.
