Park Place of Elmhurst is an elegant five-star community conveniently located across the street from Edward Elmhurst hospital. Here at Park Place of Elmhurst, we offer a continuum of care with the security of lifecare. Our residents also have access to assisted living, short-term rehabilitation, skilled nursing, and memory support right on campus. We are seeking a hardworking full-time LPN to support carrying out our mission to deliver quality care to our residents!
Hours available: 7am-3:30pm
*Must be eligible to work two holidays a year and two weekends a month.
Summary of essential job functions
- Responsible to provide direct nursing care to the clients and manage the day-to-day activities performed by the Certified Nursing Assistants working as a team.
- Ensures that client needs are met within professional standards of practice according to physician orders.
- Administers medications and treatments according to physician orders.
- Assists clients in meeting their goals through assessment and monitoring of clients care needs and interaction with the interdisciplinary team including the physician.
- Provides education and guidance to clients and their families on disease process and management.
- Manage the CNAs in accordance with current rules, regulations, guidelines, policies and procedures established by PLS.
- Document in nurses’ notes in an informative and descriptive manner that reflects the care provided for the resident, as well as the resident’s response to care.
- Identify the resident before administering any medication or treatment.
- Ensure adequate medical supplies are available to meet the medical needs of the patients. Report the needs to the Director of Nursing and/or Administrator.
- Provide direction and leadership to the CNA staff.
- Receive and review nursing report at the beginning of the shift and compile nursing report at the end of the shift.
- Asses the residents change in medical condition to the proper authority at that time.
- Ensure the care given is in accordance with federal and state regulations, as well as the resident’s wishes.
- Monitor vital signs in accordance with the resident’s treatment plan.
Requirements
- Must be licensed as LPN in the State of IL
- Previous experience in long-term care is a plus.
- Minimum one year of medical/surgical experience.
- The LPN must be a person who is licensed as a Licensed Practical/Registered Nurse in the State of IL and be in good standing.
- The nursing license must be active and unencumbered in accordance with the department and professional regulations.
