We are in search of a physical therapist who is willing to travel to various home settings to plan and implement prescribed physical therapy activities based on diagnosis and objective assessment given by the physician. Create a physical therapy program designed to aid patients in reaching their maximum functional ability, highest level of independence, and/or a reduced level of pain, injury, or disability.

• Has a lymphedema certification.
• Have an active and cleared licensed as a physical therapist issued in the State of Florida.
• Active and valid driver license and auto insurance.
• Have at least one year of Home Care experience with little to no supervision.
• A minimum of a Bachelor of Science or obtained a Master of Science degree from an accredited school of physical therapy.
• Have experience in long-term care; knowledge of federal and state long-term care regulations; and knowledge of Medicare regulations and reimbursement requirements.
• Must be able to lift, push, pull, and stabilize patients during exercise sessions.
