Description

Position Summary:   

The part-time Job Coach provides job support, training, and consultation for individuals with Intellectual/Developmental Disabilities working in the community.


Position Responsibilities:

  

  1. Performs task analysis on each new job at a community work site and orients the individual to the new work site.
  2. Assists the employer in training and coaching the individual on each aspect of the job and all skills necessary to perform      the job.
  3. Requests job accommodations with the employer if necessary. Promotes natural supports.
  4. Provides one-on-one support on the job and creates a fade out plan to lead to independence.
  5. Individual case management to include case notes, job coaching reports, and other employment related documentation.
  6. Works with Employment Specialists to implement each individual’s IPE—Individualized Plan for Employment.
  7. Maintains regular communication with the individual and employer, on-site and off-site.
  8. Provide support to individuals in making healthy lifestyle choices, which will positively affect the individual’s work life.
  9. In consultation with the Community Employment team, identify and assist the individual in facing and devising strategies      to overcome barriers to employment.
  10. Researches and provides information about public transportation or ride share options, provides training, as necessary.

Required Education & Experience:

- HS Diploma or GED equivalent required.

- 21 years of age or older


Additional Licensing/Certifications: N/A

- Current Illinois Driver’s License, valid and acceptable driving record


Required Skills, Knowledge & Abilities:

- Excellent oral and written communication skills.

- Ability to deal tactfully and professionally with employers to support the individual’s success.

- Ability to maintain confidential information.

- Schedule may vary, requiring flexibility and may include some weekends and evenings.


Preferred Skills, Knowledge & Abilities:

- Experience supporting individuals with intellectual/developmental disabilities on  in the workplace.

- 1 year of related experience 

- Knowledge of job coaching methods 

Salary Description
$17.00 an hour
