Summary: To promote learning and growth for all people that will enhance their quality of life. Collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an person’s and their family comprehensive needs and desires. Case Manager must know and use “Person First” language. They must understand and believe that people have the right to participate in the decisions that affect their lives. They must seek and honor individual preferences. They must demonstrate the creation of an environment that promotes a “Person Centered Life”. They must demonstrate positive interactions with others
Essential Duties and Responsibilities include but are not limited to the following:
? ISP- Plan Meeting – (preparation) develops, write and implement plans
? Employer Check-Ins/audits
? Job/Volunteer Searching; Community based activities
? Maintains communication with all necessary parties – internal and external.
? Linking with Resources
? Maintains Customer Service with all parties
? Consistent, timely, effective communication with Family, etc.
? Consistent, timely, effective Communication with Employers
? Consistent, timely, effective engagement with people we support
? Developing and maintain Natural Relationships
? Routine check Ins with caregivers
? Conducts Unannounced Visits
? Conduct Audits
? Monitors and audits Documentation
? Utilize data tracking system to monitor progress
? Communication with employers/families/staff to monitor satisfaction and progress
? Document incidents
? Document in data tracking system and review documentation
? ISP Plan Implementation and Updates – obtain necessary information and documentation
? Arrange/provide Transportation
? Educate staff on People Schedules/Communication Boards
? Conducts Trainings- BP, Seizures, Goals, Fundamental Rights, etc.
? Facilitates collaboration with other entities

Adaptability – Must have the ability to analyze programming needs and implement alternate approaches, as necessary. Ability to manage daily change and modify personal schedules based on the needs of the person and organization.
Time Management/Organizational – Must have the ability to effectively plan, prioritize, manage work assignments; complete tasks, projects and reports on time; must be able to prepare all necessary documents and correspondence for upcoming meetings in advance with accuracy and efficiency.
Facilitation/Negotiation - Must have the ability to conduct meetings, ensure team consensus, and facilitate the desired outcomes for the benefit of the person(s). Collaborate with all entities of the person’s life in a multidisciplinary approach. Must assist and support each person in taking a lead role with the development of their plan.
Problem Solver/Critical Thinker - Must have the ability to identify, organize, analyze all aspects of the situation skillfully and resolve in a timely manner. Must be able to develop possible solutions and organize all the necessary entities for a timely resolution. Must be able to modify based upon circumstances.
Communicator – Must have the ability to communicate clearly and effectively both written and orally and with varied audiences. Must listen actively to all cues and modes of communicating. Must have the ability to obtain and share information in a succinct and organized manner.
Education and/or Experience
Bachelor’s Degree required.
To perform this job successfully, an individual must have knowledge of Microsoft Office Suite Database software (Word, Excel, and Outlook). Experience in Therap, PCIS2 and effective use of agency and state systems, etc.
