LPN Charge Nurse (PRN)
Description

The primary responsibilities of the Staff Nurse are to provide quality, professional health care for all residents of Assisted Living. This includes administering medications in a safe manner and within regulation requirements, responding to special needs and emergencies and providing supervision for the Medication Aides and Certified Nursing Assistants (C.N.A.) in collaboration with the DON.  

  • Must be a licensed practical nurse. 
  • Must be a graduate of an accredited school of nursing and licensed through the Virginia Board of Nursing. 
  • Supervisory skills required; experienced preferred. 
  • CPR certified or the ability to attain.
  • Strong customer service skills; maintain a hospitality approach to engaging with all individuals.   
Requirements
  • Administer medication(s) per unit dose system within regulation time frame and without error. Complete the medication pass   within regulation timeframe. Document/sign all medication administration records daily. 
  • Complete monthly summaries on each resident as scheduled. Meet with nursing assistants to determine requirements and all areas are completed accurately on the day scheduled for completion. 
  • Supervise Medication Aides and Certified Nursing Assistants to ensure that resident-centered care is provided. 
  • Complete and/or supervise all treatments using standard nursing protocol. 
  • Record nursing observations on residents’ charts. Chart, date and sign after each entry. Record daily or as changes are   necessary. 
  • Transcribe, analyze, comprehend and implement doctors’ orders accurately and notify appropriate personnel of changes.
  • Complete dietary and maintenance forms accurately as required. 
  • Update daily flow sheets for C.N.A.’s so that all services rendered are in agreement with the Care plan. Ensure all information is current and clearly noted for C.N.A.’s to implement. 
  • Report all significant resident issues to the Unit Manager. Report all issues as soon as noted or within a timely manner. Utilize 24-hour report sheet to report all changes and orders.  
  • Respond to and implement fire plan for Healthcare Center residents according to procedure. Ensure resident and staff safety. Assume the command position until the fire department arrives. 
  • Maintain knowledge of safe operation of oxygen, IV’s, suction machines, catheters, etc. Use proper techniques when working with medical equipment. 
  • Practice universal precautions and ensure that C.N.A.’s always use universal precautions. 
  • Complete admissions, transfers, deaths and discharges. Ensure that charting is in compliance with standards set forth by   licensure and Sunnyside while maintaining residents’ safety and security. 
  • Prepare change of shift report and effectively communicate with oncoming staff to ensure continuity of care for residents. 
  • Make rounds on each shift to account for all residents, assess the building for safety and provide medical attention as needed.
  • Handle emergencies and/or stressful situations with professionalism. 
  • Draw labs as ordered and report them to physician in a timely manner.  
  • Assess complaints/issues with resident and notify physician as needed. 
  • Make responsible parties aware of changes with resident care as needed and in a timely manner. 
  • Maintain clean, organized and orderly work areas. 
  • Meet all training requirements as required by Federal and State Regulations, company and community policies. 
  • Demonstrate a commitment to safety by following safety rules and guidelines; follow good housekeeping practices, take proper care of equipment and perform job duties and responsibilities in a safe and responsible manner. 
  • Demonstrate dependability, reliability and punctuality. Report to work when schedule, ready to work.
