The Emma L. Bowen Community Service Center is seeking a Case Manager to join our team. This position is a terrific opportunity for someone to gain additional experience and make a difference in the lives of our clients.

For over 45 forty-five years, our organization has provided mental health and holistic services in northern Manhattan for families, children, adolescents, and other adults including the elderly in a supportive and caring environment. We also have an out-patient and residential substance abuse program as well as a Therapeutic Preschool for 3-5 year olds. 

Upper Manhattan Mental Health Center is a Certified National Health Service Corps (NHSC) site.

                                                                                                     JOB DESCRIPTION


Title of Position:  Children's Care Manager

Department:  Children's Care Management Program

Hours:   Full-time, Monday-Friday, 35 hrs./week

Union:  Union Position

     

Essential Duties and Responsibilities:


  • · Manage a caseload of 12-25 clients.
  • · Connecting client   with needed medical, mental health, and/or substance use services to reduce   ED/ER visit and readmission.
  • · Meeting with   school counsellors and educational team to address IEP needs.
  • · Collaborating with   clinicians to identify wrap around services for clients in UMMHC’s outpatient   behavioral health and addiction treatment programs.
  • · Connecting clients   with services including insurance, housing assistance, educational and   vocational training, supported employment, specialty medical care and other   social service needs.
  • · Documenting all   client interactions and keeping clients’ clinical teams updated on all   efforts via weekly meetings.
  • · Researching   outside social service supports to identify potential resources for clients.
  • · Accompanying   clients to appointments and performing home visits as needed
  • · Contributing to   case conferences
  • · Organizing group   trainings to support clients as they work to integrate into their communities   and lead productive, satisfying lives.
  • · Other   responsibilities as determined by the program director.
  • · Personal   Qualities: This is an ideal position for someone with excellent interpersonal   communication skills and the ability to work collaboratively with other   disciplines. 


Education and/or Work   Experience Requirements: 


  • Bachelor’s Degree in Human Services, Healthcare, or related discipline
  • Minimum of two years in the human service field. 
  • Must be able to write psychosocial, participate in team conferences, plus work as part of a team. 
  • Must be able to work independently.
  • Bilingual: English/Spanish


Salary range: $50,000
