Become part of the Coordinated Access Navigation (CAN) team! Elica is currently recruiting for 7 Coordinated Access Navigators! 


Coordinated entry is a streamlined system that’s designed to match people experiencing homelessness with available housing and shelter options. This process helps prioritize resources based on vulnerability and severity of service needs to ensure that people who need help the most can receive it in a timely manner.

The ultimate goal is to improve health and housing outcomes in our community by supporting people with finding the most appropriate housing and supports. These include preventing experiences of homelessness before they begin locating permanent housing.


WHAT YOU'LL DO:

 The CAN Team provides services such as housing problem-solving, system navigation, and limited financial assistance. The Coordinated Access Navigator (CAN) plays a critical role in the Coordinated Access System (CAS), by increasing accessibility to resources and supporting households to resolve their housing crises whenever possible, with the goal of reducing the number of households who experience homelessness and housing instability.

Requirements


WHAT ARE WE LOOKING FOR?

The successful candidate will be willing and able to:

 

  • Coordinate, integrate, and leverage resources to maximize impact of services for individuals who are experiencing (or risk) homelessness;
  • Assess housing barriers of individuals and families experiencing (or risk) homelessness to determine housing and service needs;
  • Support youth and young adults in Sacramento County to successfully exit homelessness;
  • Engage in constant communication with 2-1-1 Sacramento and community partners to identify housing search needs and barriers;
  • Accept referrals or waitlisted clients from 2-1-1 Sacramento and Contact client within 24 hours of a 2-1-1 Sacramento;
  • Assistance with connection to resources like housing stabilization services, financial assistance application completion, and other housing-problem solving services;
  • Assist with navigating resources available in the community, from Health Plans and at Elica Health Centers;
  • Caseload of 15-25 homeless (or risk of homelessness) clients/patients;
  • After receiving training from Elica’s Workforce Development and Department of Social Care staff, Elica’s CAN navigators accompany both Elica’s mobile health clinic staff and our on-foot street medicine provider teams to conduct outreach to potential homeless clients/patients at shelters, encampments and other locations within Sacramento County;
  • Outreach activities within homeless (or risk of homelessness) communities;
  • Prompt responses to inquiries, emails, calls and timely documentation and reporting;
  • Performs other related duties as assigned.

The successful candidate has:

  • High school diploma or above
  • Proficient in HMIS and/or EHR.
  • One (1) year of working or personal experience related to the homeless population and/or with coordinated entry systems OR
  • One (1) year of working or personal experience working with youth experiencing homelessness and housing instability, and/or have had experience in foster care, probation, mental health systems.

Is skilled in:

  • Written and oral communication, including facilitating meetings and documentation;
  • Human relations skills including the understanding of and respect for boundaries, values, and personal differences;
  • Organization and time management including meeting deadlines and prioritizing competing activities.

Has the ability to:

  • Identify essential factors in situations, creatively problem solve, and actively engage individuals in solutions;
  • Easily establish contact with participants who are experiencing homelessness;
  • Develop and maintain receptive, open, and approachable lines of communications with clients, social service providers, community providers and county staff;
  • Establish and maintain positive working relationships with others;
  • Be flexible, think creatively and explore new alternatives;
  • Take a task from start to finish and follow up with others to ensure the task is complete.

Additional Requirements:

  • Must have a current and valid California driver’s license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours.
  • If selected for an employment opportunity with Elica Health Centers, external hires must provide proof of immunizations (Hepatitis B, MMR, Varicella & Tetanus), tuberculosis clearance, and proof of COVID-19 vaccination status* prior to their scheduled start date. Please be advised that this position is subject to criminal background investigation and drug screen.
  • Work schedule: 8-hour shifts (starting at 8 a.m. or until 8 p.m.) 5 days a week (Monday-Friday or Saturday and 4 other days during the week)

 

Physical Requirements and Work Environment:


The work environment is characteristic of a medical/dental clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
