Job Description:
•Work collaboratively with multidisciplinary teams to optimize drug therapy.
•Review and interprets medication orders and prescriptions for inpatients to ensure dosage, route, frequency, potential interactions, and allergies have been screened.
•Provide clinical pharmacy services to hospital patients including, but not limited to order review and verification, medication reconciliation, pharmacokinetic dosing/monitoring, renal dose adjustment, anticoagulation monitoring, and IV to PO conversion.
•Perform medication distributive functions, including, but not limited to, preparation and/or verification of medications, labeling, compounding, and dispensing. Provide drug information to prescribers, hospital staff, and patients.
•Maintain established procedures concerning quality assurance monitoring, security of controlled substances, and disposal of hazardous medications.
•Oversee technician activities and performance.
•Provide direction and guidance, as needed, to pharmacy technicians.
•Participate in orientation, instruction training of pharmacy technicians, residents, pharmacists, and other health professionals.
•Performs duties documented in accordance with requirements of all regulatory and governing bodies of pharmacy and/or the hospital.
•Prepare the full range of sterile products and performs pharmaceutical calculations for a variety of preparations.
•Provide for the safe handling, preparation, storage, and disposal of and hazardous drugs.
•Ensure strict adherence to sterile procedures and correct operation and maintenance of equipment.
•Receive and reviews, prepares, and fills physician orders for intravenous (I.V.) solutions, other parenteral, inpatient unit dose orders, prescriptions, and bulk drug orders.
•Perform final pharmacy check on sterile products, bulk drug orders, replenishment of emergency medication carts and inpatient automated dispensing
PSYCHIATRY DEPARTMENT PHARMACIST: A minimum of one-year pharmacy experience within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency is required. ASHP accredited PGY-2 Specialty Residency in Psychiatry or Psychiatry Pharmacy Fellowship or equivalent experience required. Board Certified Psychiatric Pharmacist (BCPP) is preferred. Chief, Department of Pharmacy has to waive requirements.
