InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:


Provides patient care by conducting patient assessments, administering treatment and medications, performing diagnostic tests, arranging patient follow-ups, and providing client education about the disease process. Records medical history and symptoms, completing documentation in the medical record.


Essential Duties & Responsibilities:

  • Develops curriculum and facilitates educational groups.
  • Performs selected nursing administrative duties and assists providers as part of the clinical care team.
  • Perform phlebotomy; assists with EKGs, exams, and office procedures as needed; provides other patient care services as directed by the providers.
  • Identifying client needs, current services, and available resources, connecting the client to services and resources to meet established goals by the treating provider.
  • Will perform patient triage both in person and via telephone.  
  • Assess and identify patients' needs, then implement and monitor the patient's medical plan and treatment via the complex care management processes. 
  • Perform physical examinations and diagnostic tests
  • Counsel and educate patients and their families on treatment plans.
  • Administer medication, change wound dressings and care for other treatment options.
  • Consult with (D)PM and providers to decide on the best treatment plan for patients.
  • Lead and prepare care team huddles. 
  • Research ways to improve healthcare processes and improve patient outcomes
  • Performs all other duties as required.
Requirements

Education &/Or Experience:

Graduation from an accredited nursing program. Current State of Connecticut RN licensure required. One (1) year medical experience in a public health setting required.
