Adult & Child Health is an integrated healthcare provider in Central Indiana. Providing high-quality, affordable care including primary care, behavioral health services, substance abuse treatment, mobile crisis response, community-based services, homeless outreach, school-based treatment, and more.


About this role: As a community Skills Development Specialist you will work with adult clients with chronic mental illness to build skills that help them be more independent and/or maintain independence. Community Skills Development Specialists help clients access and utilize available resources. You will coordinate and communicate with clients’ families, as well as internal and external providers.


The Location and hours: A&C is based in Central Indiana. For this position, we service the following county: Marion

Hours are M-F 8-4pm or 9-5pm with flexible scheduling available. Some ON CALL needed.


Responsibilities include but not limited to:

  • Skills training 1:1 with client &/or family members, in home and community
  • Coordination of care with internal and external providers
  • Complete quarterly treatment plan review with clients
  • Manage and maintain clients’ charts and perform scheduled assessments
Requirements
  • Bachelor's degree in behavioral health or related field
  • One (1) year of work-related behavioral health experience (internships and/or volunteer experience can count toward one year experience requirement)
  • Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
