Who we are looking for:

  • The development and execution of a network provider and facility engagement strategy aimed at building collaborative relationships and referral flows with network PCPs, specialists, and facilities. Your work will ensure ConcertoCare teams are able to effectively and efficiently communicate with network providers and gain access to attributed patients across settings (e.g., SNFs, EDs). You will be essential to building an ongoing presence in the provider community to foster future business growth.
  • Reporting to the Director of Engagement, the Director, Provider Relations is responsible for:
  • Own and maintain relationships with key health plan stakeholders
  • Build strategy in partnership with health plans to drive provider engagement
  • Develop, own, maintain, and grow relationships with key accounts (Health Systems, ACOs, IPAs, Physician Groups)
  • Develop KPIs and metrics for the provider relations team
  • Work with key stakeholders to develop internal and external reporting to measure KPIs and organizational success within provider relations scope
  • Direct, manage, and supervise Physician Liaison (s) & Sr. Physician Liaison (s)
  • Ensuring timely and accurate documentation of outreach by the provider relations team


What you will do:

  • Interface, collaborate and influence executive leadership and key stakeholders, both internal and external
  • Develop network provider and facility engagement strategies
  • Conduct outreach for new business opportunities both telephonically and in-person
  • Engage with Health Systems, IPAs, ACOs, Physician Groups & PCPs and other providers to build collaborative relationships
  • Educate key account stakeholders on ConcertoCare products and services
  • Utilize reporting and KPIs to track activity and key network information
  • Continually identify and implement opportunities to improve network provider engagement defined by collaboration with ConcertoCare clinical teams and patient referrals into ConcertoCare clinical programs
  • Improve network facility (e.g., hospital, SNF) engagement as defined by ConcertoCare clinician access (in person and / or telephonic) to attributed patients and opportunities for inpatient and discharge care collaboration


What you will need:

  • Bachelor’s degree required
  • Master’s degree preferred
  • 10+ Years Provider Network Management / Provider Engagement Experience
  • ACO/Health System and / or Medicare Advantage health plan experience 
  • 3+ years’ experience using a CRM (preferably Salesforce)
  • 3+ years’ experience working with key stakeholders at health plans
  • Advanced analytical skills
  • Excellent verbal and written communication skills
  • Ability to travel within the east coast of United States, up to 50 % of the time
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 5 years.  No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years.  Multiple violations and accidents within the last 5 years would be subject to review.

Base Salary/ Wage Range $120,000 - $135,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
