Licensed Practical Nurse (LPN) – Weekend On-Call

The Weekend On-call LPN is a member of the Physician Housecalls team. In that capacity they are expected to be familiar with the philosophy, goals and objectives of the Company and work collaboratively to achieve them. They must be cognizant of, and comply with, all Physician Housecalls policies and procedures, as well as pertinent state and federal regulations. They are expected to work on-call from Friday - Monday (5pm - 8am) on a rotating basis to support our teams across the states of Kansas & Oklahoma; this is a remote position.


Essential Job Responsibilities

  • Works remotely as first call responder to triage urgent calls to the on-call provider
  • Knowledge of nursing theory and practice and appropriate medication dosages
  • Ability to deal with a variety of personalities and situations
  • Knowledge of Federal and State regulations as well as agency policy and procedures to provide excellent customer service for patients.
  • Integrity to maintain confidentiality of information in compliance with HIPPA and complies with OSHA and company standards
  • Demonstrates familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies
  • Works independently but performs as a member of the team
  • Demonstrates skill in nursing practice, ability to cope with patient and family emotional stress and tolerance/respect for different family circumstances
  • Documents interventions on nursing notes, accurately, legibly, with consistency, within a 24-hour period or next working day. Maintains current patient records as changes occur
  • Maintain regular communication with the Nursing Director as appropriate Notifies Nursing Director of unusual or potentially problematic patient/family issues or change of condition
  • Provides input to the plan of care Is accountable for proper utilization of the patient’s plan of care
  • · Seeks to control cost related to the patient’s care such as, labs, procedures, supplies, medications, etc.
  • Participates in agency orientation and in-service training programs for professional staff
  • Takes physician verbal orders or phone orders, receives faxed physician orders, documents & communicates new orders to patient’s assigned nurses, updates medication profiles
  • Receives home health patient lab results, faxes them to physician and notifies the Nursing Director
  • Assists with answering telephone calls, processing of referrals, and coordination of patient care, as requested with effective oral and written communication.
  • Perform all other duties as assigned


Requirements

Qualifications:

  • High school diploma/ GED
  • Graduate of an accredited school of professional nursing and licensed to practice as an LPN in the state in which they work and eligible for multi-state licensure.
  • At least one year of experience as a staff nurse in a clinical setting.

Experience Requirements:

  • Excellent communication skills, both written and verbal, are required.
  • Ability to work independently and demonstrate initiative is required.
