Klarus Home Health is seeking a full time, part time, or PRN dedicated and professional Physical Therapists to join our team serving our patients currently enrolled in our home health services in the Parker, Castle Rock, HIghlands Ranch, Lone Tree, CO and surrounding areas.
This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.

Key Responsibilities:
- Develop and implement individualized physical therapy treatment plans.
- Evaluate and document patient/client progress, adjusting treatments as necessary.
- Coordinate care with other healthcare professionals to ensure comprehensive treatment.
- Educate patients/clients and their families on treatment plans, home exercises, and preventative care.
- Maintain accurate and timely patient records in compliance with company and regulatory standards.
- Utilize durable medical equipment as needed and provide training to patients/clients.
- Ensure patient safety and comfort during therapy sessions.
- Participate in continuous professional development and stay updated with the latest therapy practices.
Job Qualifications:
- Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
- Licensure: Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, or meets one of the requirements listed in 42 CFR §484.115(h).
- Experience: At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
- Skills: Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.
- Transportation: Reliable transportation, a current Driver’s License, and valid and current auto liability insurance.
Environmental and Working Conditions:
- Work in healthcare facilities or patients’/clients’ homes in various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally.
- Some exposure to unpleasant weather conditions.
Physical and Mental Effort:
- Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing and walking.
- Must be able to address patient/client and family psychosocial needs effectively.
- Requires good hand-eye coordination and manual dexterity.
