Job Summary:

As a Lead Caregiver aka Care Captain, at Careworks Health Services, you will play a crucial role in maintaining and enhancing the quality of care provided to clients by supervising and supporting caregivers. This position requires a combination of clinical expertise, managerial skills, and a commitment to excellent care standards.


Key Responsibilities:

  • Training and Support: Guide and mentor caregivers in the field to ensure they deliver high-quality care consistent with established standards. Conduct hands-on training sessions and provide continuous feedback.
  • Quality Assurance Visits: Regularly schedule and conduct unannounced visits to clients to monitor and ensure the quality of care provided. Assess and address any deviations from the care plan.
  • Backup Caregiver: Act as a substitute caregiver for last-minute shift cancellations, ensuring that client care remains uninterrupted and of high quality.
  • Record Maintenance: Keep detailed and accurate records of visits. Prepare and submit reports on quality assurance activities and outcomes.
  • Issue Resolution: Promptly respond to and resolve any issues or complaints raised by clients or their families. Act as a mediator to facilitate communication and problem resolution between staff and clients.

WHAT A TYPICAL DAY LOOKS LIKE:

AM

  • Cover canceled shifts: Respond to any immediate caregiver call-offs, ensuring clients receive uninterrupted care.
  • or Train caregivers in the field: Conduct morning training sessions with new or current caregivers, focusing on hands-on skills and care procedures. (
  • or  Onboard new clients

PM

  • Quality Assurance Visits: 2-3 day: Perform in-field quality assurance checks, ensuring care plans are followed, clients are satisfied, and caregivers are performing well.
  • Collecting and Updating Documents: During QA visits, collect timesheets, care notes, and other necessary documents from caregivers and clients.
  • Administrative Tasks: Submit collected documents, update records, and ensure all paperwork is accurate and timely.
  • Planning and Scheduling: Plan and schedule the next day’s QA visits, training sessions, and potential shift coverage.


Performance Metrics:

  • Visit at least 30 clients each month (with 100% private pay and veteran clients visited)
  • Conduct 5 training sessions per month 
  • Ensure 100% of new clients have care plans binders by first week
  • 100% Care binders have updated care plans
  • Client and caregivers: 90% satisfaction scores
  • Increase billable hours by 20 hours each month.
  • 4 new client referrals per month from visits
  • Client and caregiver satisfaction: 90%
  • Shifts covered due to call-offs: 100%
  • Response time to cover shifts: <1 hr
Requirements

Qualifications:

  • At least 5 years of experience in home care or a related field.
  • Active CNA or HHA license
  • Great communication and people skills.
  • Ability to train and mentor caregivers.
  • Flexible with scheduling and able to work in a fast-paced environment.
  • Valid driver’s license and reliable transportation.

Preferred Qualifications:

  • Bilingual
  • Experience with electronic health records (EHR) systems.
  • CPR and First Aid certified.

Salary Description
