JOB TITLE: CARE EXTENDER


Minimum Education

   A high school diploma or equivalent.


I. Special Qualifications

  1. Have lived experience with I/DD or TBI with a demonstrated knowledge of and direct personal experience navigating the NC Medicaid service delivery system; or
  2. Have lived experience with behavioral health condition and as a Certified Peer Support Specialist.
  3. Be a parent or guardian of an individual with I/DD, TBI, and/or behavioral health condition and have at least two years of direct experience providing care for and navigating the Medicaid service delivery system on behalf of that individual; or
  4. Have at least two years of paid experience performing the Care Extender functions, with at least one year of paid experience working directly with the Tailored Care Management eligible population.

II. Responsibilities

  1. Collaborate with Care Manager to conduct initial and ongoing general outreach and engagement activities with members.
  2. Support Care Managers in member service needs and address unmet health-related resource needs.
  3. Links members to social services and community resources for housing, food, employment, and other areas.
  4. Track linkage referrals for community-based resources and social support services to close the loop and monitor needs being met.
  5. Assist members by facilitating the scheduling appointments with providers; arranging transportation for healthcare visits; getting prescriptions filled; and following up with members.
  6. Participate in care team case conferences, team huddles, meetings, and planning activities.
  7. Assists in identifying members' needs from ongoing member contact and support activity.
  8. Assist in developing a library of materials and resources to help educate members on issues and options such as health promotion, self-advocacy, navigating service systems, guardianship options/alternatives, employment, success in school, and family planning.
  9. Support member engagement in health promotion activities.
  10. Participate in educating members about mental or physical health, abuse, violence prevention, medication, and available community and social resources.
  11. Encourage members to develop work skills and participate in social, recreational, or other therapeutic activities that enhance interpersonal skills and develop social relationships.
  12. Support Care Manager in follow up and obtaining reports, records, and other information from the extended care team, medical providers, long-term support providers, and others.
  13. Maintain accurate documentation of contacts and activity within the care management system.
  14. Delivers services according to applicable laws, regulations, policies and procedures, supporting the organization's corporate integrity efforts by acting ethically, reporting known or suspected violations of applicable rules, and cooperating fully with all organizational investigations and proceedings. Ensuring health information management requirements are met.

I. Recruitment Standards

  1. Basic knowledge of NC Medicaid and Innovation Waiver services.
  2. Proficiency in Microsoft Word, Google Suite, Excel, and navigating the internet.
  3. Strong data entry experience.
  4. Must be detail-oriented and able to multitask.
  5. Must be able to work in a fast-paced environment and have a strong sense of urgency.
  6. Flexibility and adaptability to support members.
  7. Ability to organize and prioritize multiple tasks and priorities.
  8. Ability to exercise strict confidentiality and sound judgment for sensitive member information, maintaining the integrity of members.
  9. Ability to work cooperatively and effectively with others to set goals, resolve problems, and make decisions to meet members, team, and agency needs.
  10. Ability to develop and maintain partnerships with others inside or outside the agency.
  11. Strong interpersonal skills and excellent listening, verbal, and written communication skills.

II. Physical Requirements

  1. Activities: Sitting, walking, typing, talking, hearing
  2. Activity Level: Moderate
  3. Visual Acuity: Requires visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; and/or extensive reading.
  4. Working Conditions: The worker is not substantially exposed to adverse environmental conditions when performing typical office or administrative work on-site.
  5. Travel: 50% (variable to change). Reliable transportation is required.

III. Hours/Days of Work

8:00 a.m. to 5:00 p.m., Monday through Friday (Exempt). The weekly schedule must be flexible to meet the needs of the agency and families served. Full-time Regular position. Rotation in on-call coverage required.
