About the Role

As the RN Care Manager, you are a part of a revolutionary service coordinating all patient care between primary, specialty, inpatient and behavioral health services. You serve a variety of patients including individuals with substance use disorders and both children and adults with mental illness. 

Your role is to assist clients in initial and ongoing care coordination including:

· Monitoring client’s status with ongoing assessment and screenings.

· Collaborating with the client to develop an initial care plan.

· Identifying strategies to implement evidenced-based wellness and prevention initiatives.

· Communicating with medical providers, substance abuse service providers, long term care and hospitals.

· Providing education on health conditions, treatment recommendations and medication.

Requirements

         

  

Qualifications

· Michigan Registered Nurse (preferred) or Licensed Practice Nurse with:

o A minimum of one year’s experience in a managed care setting preferred and,

o A minimum of three to five years medical nursing, surgical nursing and/or nursing in the acute care setting.

· Valid Michigan’s driver’s license.

· Ability to sustain State Licensure in field of practice and/or Certifications to perform duties of the job.

· Ability to be certified in First Aid and CPR within 90 days of hire (agency available training)

· Knowledge of chronic physical and mental health conditions, substance use disorders, evidence-based guidelines, prevention, wellness, health risk assessments, and patient education. 

· Ability to successfully navigate a computer system with minimal support.

Preferred Skills

· Training and education in Substance Use Disorders, Behavioral Health, and Co-occurring disorders. 

· Experience in motivational interviewing for client engagement along with other interventions, screenings, and treatments used in behavioral health and substance use disorders.

· Proficiency with Electronic Health Records (EHRs), data entry, and computer software including Microsoft (office, word, excel, power point, and teams).

Work Environment:

CWN prioritizes work and life balance. Your supervisor will work with you to utilize flex time and paid time off. You will primarily work in the office with the following schedule:

· Monday through Friday

· Between 8:00am and 5:00pm

You may be required to work hours outside of the regular schedule if assigned by your supervisor to meet clients’ needs.

Light travel (<5% of time) may be required for job-related assignments, training or presentations.
