Pay Rate Starting at $39.00/hour

Clinical Coordinator - Full-time 

The Clinical Coordinator functions as an assistant to the Nurse Manager with unit organization and the clinical management of resident care. The Clinical Coordinator will collaborate with the Nurse Manager to establish and communicate goals with all staff, as well as monitor and measure outcomes. 

 


ESSENTIAL JOB FUNCTIONS

  

General:

  •  Is familiar with the Church Home mission statement and nursing department philosophy and goals.
  • Is familiar with and able to interpret and enforce all policies and regulations.
  • Consistently complies with established policies and procedures.
  • Assists with implementation of quality assurance programs and monitors completion of audits, pertinent to unit.
  • Utilizes supplies in a cost-effective manner.
  • Reports unsafe condition(s) to security, maintenance as soon as possible.
  • Is familiar with established emergency procedures and staff incident procedures.

Clinical:

  • Uses the ANA standards of gerontology nursing practice as a framework for professional responsibilities.
  • Assesses and identifies resident needs and communicates to medical team, Nurse Manager and other disciplines as appropriate. Ensures follow up is complete.
  • Prepares and participates in resident rounds with the medical team. Provides the medical team with orders for renewal and approval at specific times.
  •  Is responsible for the follow up of routine clinical care for residents.
  • Assists with development, implementation and documentation of care plans and MDS.
  • ·Responsible for ensuring skin team requirements are completed. Familiar with all components of the skin integrity policies and procedures.
  •  In collaboration with the Nurse Manager and Social Work Dept., notifies responsible parties of change in resident condition.
  • Administers medications per medication policy and procedure.
  • Orders medications from the pharmacy per policy and procedure.
  • Transcribes and implements orders per policy and procedure.
  • Maintains resident’s immunization schedule per policy and procedure.
  • Follows established documentation procedure(s).
  • Is familiar with documentation requirements specific to MDS.
  • Complete assessments within allotted timeframe.
  • Updates care plans, care card as needed.
  • Documents all incidents/accidents and follows through per policy/procedure.
  • Monitors nursing staff for use of proper body mechanic and equipment when moving/transferring residents.
  • Is assigned On-Call responsibilities on a rotating basis per nursing department needs.
  • Follows nursing department policy in regards to dress code.

Assistant to Nurse Manager/Leadership:

  • Is a role model to staff. Builds morale and encourages/demonstrates a culture of teamwork.
  • Assists with implementation of administrative policies established by the Board of Directors.
  • Assists with assigning nursing staff to resident care activities and other duties.
  • Assists the Nurse Manager with supervision of professional and non-professional nursing staff.
  • Provides input to Nurse Manager regarding employee performance.
  • Keeps Nurse Manager aware of issues relating to nursing procedure and resident care on a timely basis.
  • Assists with monthly staff meetings on unit.
  • Assists with resolution of problems/issues with resident/family/staff relations at lowest level of management and keeps written records.
  • Conducts unit activity in the absence of Nurse Manager, seek guidance as needed.
  • In absence of the Nurse Manager, attends interdisciplinary care planning committee, morning report and other designated meetings.
  • Monitors daily documentation of nursing staff for accuracy and completion.
  • Assists the Nurse Manager with providing medical team with pertinent resident information via medical communication form as needed.
  •  Assists with unit orientation of new nursing staff.
  •  Assists with assessing skills and educational needs of nursing staff; communicates identified needs to Nursing Education Coordinator and Nurse Manager.
  • Collaborates with Nurse Manager to provide opportunities for staff to participate in actualizing goals and plans of nursing department.

 

Requirements

  

MINIMUM QUALIFICATIONS: 

  • Completion of an approved program of professional (registered) nursing and an unexpired/unencumbered NYS RN license required.
  • A minimum of 2 yrs clinical RN experience required; preferably in gerontology.
  • Demonstrated leadership qualities - - - acts as a mentor/role model to nursing staff.
  • Basic computer skills required including but not limited to the ability work on a laptop or PC within a Windows OS, the ability to create, open and save electronic documents, and the ability to create, save/send emails.
  • Knowledge of New York State regulatory process.
  • Effective written and verbal communications.
  • Ability to adapt to changing priorities and flexibility required.
  • Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors and the general public.

Performing duties of this job requires lifting, bending, walking and standing. Must be able to occasionally lift loads of 50 pounds without assistance and the ability to sit, talk, and hear is required. The majority of essential job duties are performed indoors as well as in resident areas where universal precautions apply. Employee may be exposed to indoor temperature fluctuations.   

SUPERVISORY RELATIONSHIP: The work is performed within established guidelines, and requires periodic supervision. The employee exercises some initiative and independent judgment to effectively perform the essential job duties of this position.

RESPONSIBILITY FOR OTHERS: Assists in supervision of nursing staff.

SAFETY RESPONSIBILITIES: Shall be fully acquainted with all safety policies and procedures of the Church Home. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.

Maintain confidentiality of all information related to the organization, residents, participants, family and staff, that may be encountered, either formally or informally, during the normal course of business. This includes medical and treatment records, financial and human resources information.
