Description

Job Title: Program Coordinator

Classification: Full Time/ Exempt

Reports to: Director of Community Health Outreach


Summary: Planning, coordination, and monitoring of fall prevention programs focused on effective strategies and interventions to help older adults decrease their risk of falling and improve their overall health.


Responsibilities:

  • Identify older adults for participation in the evidence-based fall prevention programs based on expected outcomes and eligibility criteria. 
  • Educate older adults in evidence-based fall prevention programs. 
  • Maintain accurate collection, tracking and storing of data for program outcomes, deliverables, and related reporting data for ACL sponsored projects in accordance to reporting requirements.  
  • Researches all information needed to complete enrollment and evaluation process including obtaining information from older adults, community partners, and healthcare providers. 
  • Participates in community and interagency resource groups addressing issues related to fall risks and other fall-related health problems.
  • Completion of weekly, monthly, and quarterly reports as requested by Project Director and ACL. 
  • Oversee coordination of services provided by Health Promotion Specialist to ensure interventions delivered to fidelity to increase availability of education, prevention services, and community resources. 
  • Monitor the delivery of comprehensive project components including outreach, engagement, and health education to older adults located in the service delivery area. 
  • Develop relationships with key partners and organizations to create a robust communication channel to help reach and engage target population.  


Requirements

Qualifications:

  • Bachelor’s degree in social sciences, health or other directly related field; Master’s degree preferred. Community Health Education Specialist certification preferred. 
  • Minimum 2 years’ experience with health education in community-based organizations, schools, or similar settings; preferably in a leadership position. 
  • Position requires the employee to be insurance and have a valid driver’s license.
  • Bilingual (English & Spanish) preferred.


Experience/Skills/Knowledge

  • Working knowledge of outreach and community/social services resources. 
  • Advanced computer knowledge and skills including databases/data management. 
  • Skilled in dealing with difficult situations and able to be consistently pleasant and helpful.
  • Skilled in establishing good working relationships with both internal and external customers.
  • Strong interpersonal and communication skills; ability to work effectively with a wide range of populations in diverse communities. 
  • Ability to travel up to 50% of time.
  • Collaborate effectively with community partners throughout service delivery area. 

Physical Requirements:

  • Available to work flexible schedule which may include some nights and/or weekends. 
  • Work will be performed primarily in offices, public buildings, schools, churches, medical facilities, family homes and automobiles that will vary considerably with regard to the extent of heating, cooling, or physical comforts/protections.
  • Position could require long hours of standing or sitting at a computer workstation.
  • Must be able to lift at least 25lbs and be able to set up tables, chairs, and pop canopies for events in addition to moving equipment needed for specific job assignments.
  • May encounter architectural barriers (including stairs), animals not on a leash unsanitary living conditions, extreme heat or cold and contagious disease.
