Description
The Patient Coordinator will support the implementation of a registry and database of cleft lip and/or cleft palate patients throughout Cebu and nearby provinces (Visayan Region). They work closely with various community leaders and partners in the different areas of Cebu and nearby provinces to set up registration campaigns and camps. They work closely with the Program Manager – Patient Services to ensure all cleft patients in the region are located, and are provided with comprehensive cleft care.
Primary Responsibilities:
- Set up meetings at hospitals and clinics in the regions, find point people at each health care facility.
- Access existing community health care worker structures and arrange to implement cleft awareness programs in the community.
- Set up meeting with traditional authorities in the areas.
- Manage region-wide community mobilization.
- Meet with NGOs and community-based organizations within the region to partner with them for patient recruitment or sponsorship.
- Register all the patients, take their full names, their contact numbers and where they live (region, state, city) and assist with coordinating travel to the program site.
- Organize arrivals during programs.
- Assist program coordinator with putting together patient logistics (food, shelter, water, transport, reimbursements)
- Build strong relationships with patients and assess if their needs are being met. If not, approach PC or International PC for assistance.
- Create patient return lists after patient announcement, ensure all patients have transportation home.
- Create a database of all the contacts that we have within each province.
Requirements
Experience Required (education/work):
- Bachelor’s degree in related field
- Must be a Licensed Social Worker
- Experience with managing a database
- Language capabilities (fluency in both Tagalog and English)
- Experience in community health care mobilization
- Excellent computer skills (Microsoft Office 365 experience is required)
- NGO experience is a plus
- Ability to travel frequently around Cebu and nearby provinces
