Duties & Responsibilities:
· Maintain a personal, active relationship with Jesus Christ and be a strong Christian witness in all interactions with youth, staff, donors, volunteers, and the general public so as to uphold Black Mountain Home’s mission of Glorifying God by caring for children, youth, and families.
· Commit to pray regularly for the youth, staff, and ministry.
· Review and make recommendations regarding applications to the program.
· Upon intake, evaluate and assess youth needs in the following areas: general mental health, substance abuse, education, independent living skills, and medical, vision, and dental care. Based on assessment, outline specific recommendations or make referrals for treatment as appropriate and specify goals and implementation strategies to meet identified youth needs.
· Link each youth with two key support persons in his or her life that will serve as mentors while the youth remains in the program.
· Provide and/or coordinate on-going crisis management.
· Assist youth in tracking and scheduling medical appointments, arranging transportation, paying bills, and completing applications for insurance, financial aid, and school while allowing space for youth to become more independent.
· Maintain collaborative relationships with the youth, their family, and other support persons or systems.
· Through weekly one-on-one meetings with each youth in the IL Program, address issues, problem-solve, and coach youth in coping skills and/or accessing other supports as needed.
· Help facilitate monthly group meetings with IL Program youth with the purpose of addressing community concerns, fostering support networks among the youth in the program, reinforcing program expectations, and promoting healthy social, emotional and physical development.
· Help facilitate Independent Living Committee meetings twice a year.
· Complete case management responsibilities to include file maintenance in accordance with licensing standards, tracking of medical appointments and school, financial aid, and insurance applications and payment dates, documentation of the nature and duration of all services provided, and forwarding of documentation to appropriate persons or agencies.
· Help plan and conduct two annual Senior Week retreats for upcoming seniors.
· Plan and conduct weekly group enrichment activities for youth in the Independent Living Program.
· Work with Coordinator of Independent Living to conduct monthly Fire Drills and other facility inspections as needed to ensure that licensing, safety, and health code standards are being met by youth.
· Attend staff meetings, which include participation in a time of Christian devotion and prayer, and case management. Review minutes and sign off for missed staff meetings to stay aware of ministry updates.
· Participate in and supervise youth at weekly worship services at Valley Hope Church or other pre-approved Christian denominations in the local community.
· Lead Christian devotions for youth that are appropriate and relevant as requested.
· Participate in weekly supervision with the Coordinator of Independent Living.
· As part of the On-Call rotation, provide 24/7 cell phone support for all BMH programs.
· Work with volunteer groups as assigned, including weekends.
· Assist in buildings and grounds maintenance and upkeep of the Independent Living Village.
· Participate in and help lead Youth Leadership Retreats and activities as appropriate.
· Maintain buildings and grounds at the Independent Living Village with the help of IL youth.
· Coordinate and follow up with IL youth about Independent Living hours.
· Facilitate clubs as needed and appropriate.
· Coordinate discharge planning for IL youth.
· Other projects as assigned by the Coordinator of Independent Living.
Minimum Qualifications
· Must have a Bachelor’s degree in human services field with one year of experience working with older adolescents or young adults, or a Bachelor’s Degree in a related field with three years of experience working with older adolescents or young adults.

Schedule
· A normal shift is usually eight (8) days on and six (6) days off. The shift typically begins on Tuesday morning and ends the following Tuesday after Youth Leadership Team (YLT).
