POSITION PURPOSE: To provide services to in-home care clients in accordance with licensure rules, agency policy and procedures and client service planned needs and requests.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
To provide the safe provision or, or assistance with personal care tasks in accordance with OAR 333-536-0045:
a) Bathing
b) Personal grooming and hygiene
c) Dressing
d) Toileting and elimination
e) Mobility and movement
f) Nutrition/hydration and feeding
To provide the safe provision of, or assistance with medication services in accordance with OAR-536-0075:
a) Assistance with self-administration of non-injectable medication
b) Medication administration
c) Medication management
d) Medication administration through delegation in accordance with OSBN rules
To provide the safe provision of, or assistance with housekeeping and supportive services including:
a) Housekeeping tasks
b) Laundry tasks
c) Shopping and errands
d) Transportation
e) Arranging for medical appointments
f) Meal planning and preparation
g) Social contact and reassurance
- To provide client services in accordance with the service plan
- To help ensure the service plan is accurate and current
- To document services in accordance with agency policy and procedure
- To ensure client rights
- To participate in all required initial and ongoing training
- To report any accidents, injuries or suspicions of abuse or neglect
- Maintains the confidential of the work environment
- Is familiar with the current condition of each client
- Organizes tasks efficiently to deliver client care
- Relate positively to supervisor, other employees, clients and their families

QUALIFICATION REQUIREMENTS:
- Ability to facilitate clients concerns using sensitivity and confidentiality on a daily basis.
- Ability to work independently with multiple tasks, interruptions, and priorities
- Has good communication skills both verbal and written
- Has ability to maintain good working relationship with all co-workers and the public
- Successfully completes in-home care provider training program
- Ability to stoop, bend, squat and move a maximum of 40 pounds.
- Current valid driver’s license and proof of insurance coverage
NON-ESSENTIAL JOB FUNCTIONS include the following:
- All aspects of the job are essential.
UNIVERSAL PRECAUTIONS:
- Exposure to Blood/Body fluids is possible.
QUALIFICATION REQUIREMENTS:
- Successfully pass Clatsop Care Center Health District’s in-home care program training
Current CPR and First Aid Training or ability to obtain CPR/First Aid cards with in 30 days of hire.
