Description

Mission Healthcare at Bellevue is seeking a full-time Social Services Assistant to join their team! The main responsibility for this position will be Case Management/Discharge Planning for Managed Care patients.


Overview

The purpose of this position is to provide services that meet the social and/or emotional needs affecting the patient’s ability to achieve their highest level of function; and participate in the development of patient comprehensive care plans in compliance with federal, state and local regulations.


Summary of Responsibilities

  • Implement social service policies and procedures to meet patient needs in compliance with federal, state and local regulations.
  • Develop, coordinate and participate in family and patient activities designed to promote social interaction, reality orientation and intellectual stimulation.
  • Develop one-to-one professional relationships with patients and families as needed for counseling.
  • Counsel patients and families in dealing with feelings about death or dying as well as other emotional, mental, environmental or physical limitations.
  • Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.
  • Document the social service component of the comprehensive Care Plan for each patient in a timely manner.
  • Document psychosocial needs, interaction and follow-up action(s) with patient and/or family according to facility policies and procedures.
  • Assess, plan and document patient discharge needs in accordance with facility discharge planning policy and regulatory requirements.
  • Act as a liaison with social, health and community agencies.
  • Refer patients to social, health and community agencies and complete accurate documentation in patient records concerning the results of such referrals.
  • Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
  • Take part in facility in-service programs as required.
  • Organize community events related to patient’s needs and interests.
  • Explain patients’ rights to patients and patients’ representatives.
  • Participate in admission of patients as assigned.
  • Plan, schedule and implement room visits for patients unable to leave their rooms
  • Organize and participate in the facility grievance/complaint process/program.
  • Coordinate and cooperate with the activity department to plan and conduct events to meet socialization needs of the patients.
Requirements
  • Experience in social work, sociology, special education, gerontology, counseling, psychology, or a human services field.
  • 1 to 3 years of experience in a geriatric setting, preferred.
  • Knowledge of state and federal regulations for skilled nursing setting, preferred.

$25.00 - $31.08 / DOE
