Description

The Family Navigator works under the supervision of the Patient Access Manager and in conjunction with all other team members of the department and all other departments. This is a school-based service position that assists in navigating through services and programs in the community to address identified basic needs of children and their families. Supports and follows through with the vision, mission, goals, and objectives of FHH.  

    

As a member of the health center’s integrated care team the Family Navigator will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES:

  • Plays a vital role in supporting patients and families with needed resources and support.
  • Demonstrates ability to effectively communicate with a diverse population.
  • Conducts direct interviews with patients and families to complete an assessment and determine need.
  • Completes Action Plans (treatment goal formulation) dependent on assessment.
  • Work collaboratively with patients and families to create a family-driven plan that focuses on solutions and keeps the family member at the center.
  • Collaborates with a myriad of community agencies to access community services.
  • Advocates for patients and families.
  • Appropriately responds to emergency and/or crisis situations. Follows through with appropriate action, identifies appropriate person to manage incident in accordance with FHH policies and procedures.
  • Reviews and updates UDS tab in Athena, making sure all applicable areas are completed. 
  • Maintains confidentiality of patient and departmental information. Zero infractions of breach of confidentiality during evaluation year.
  • Is ready to begin work at the start of assigned shift.
  • Supports the mission, philosophy, goals, and objectives of FHH. Compliance 100% of the time.
  • Exhibits emotional maturity as evidenced by resolving conflicts by following established communication norms, expressing feelings and frustrations to the appropriate person in an appropriate place/time.
  • Clocks in and out daily, with less than four missed punches within an evaluation year.
  • Exhibits positive attitudes during times of change and disruption. 
  • Works actively to resolve conflicts. Readily accepts direction from supervisors. Supports team leader. Does not require reminders about cooperation and teamwork standards. 
  • Removes non-operating equipment from use and notifies the appropriate person when repair of equipment is needed. Monitors equipment for completion of repairs before returning to service.
  • Role models clean and neat work area. Cleans up after self.
  • Demonstrates proper body mechanics and follows safety guidelines.
  • Performs job duties in accordance with FHH policies to include infection control, standard precautions, and right to know.
  • No occurrences of using equipment for unintended purposes, including no injuries or potential injury to self, co-workers, or patients from not using equipment properly.
  • When a safety issue arises, immediately notifies the supervisor, and initiates a written report.
  • Provides direct assistance to patients and acts as a liaison with other service providers and health care professionals to ensure patients have appropriate access to the care they require.
  • Demonstrates a willingness to work with and assist others with no more than three valid complaints received by management per year.
  • Works independently and utilizes time appropriately. Exhibits a positive attitude towards work, responsibilities, FHH patients, coworkers, and community groups. Consistently demonstrates FHH values by serving as a role model to staff.
  • Consistently exhibits responsiveness to others at FHH at all levels (examples: returning telephone calls, being punctual for meetings, maintaining availability to meet, etc.).

Requirements

Minimum Qualifications:

  • High School diploma or equivalent.
  • Ability to relate to culturally diverse patients and community.
  • Computer skills and experience required. EMR experience preferred.
  • Must have excellent communication skills as well as the ability to speak, write and understand the English language. Must be able to work independently.
  • Bilingual English/Spanish language required.
  • Minimum physical effort required. Intermittent sitting with freedom of movement. Occasional walking, bending, lifting, pulling, and reaching. 
  • Normal vision and hearing required.
  • Frequent periods of concentration and attention to details with frequent opportunity for diversification of tasks. Frequent interruptions. 
  • Current and valid driver’s license and access to an automobile are required.
  • Job includes sitting, standing, reaching, trunk movements, lifting heavy objects, carrying, squatting, kneeling, crouching, walking, pushing, and pulling, which may be required for extended periods of time. Heavy lifting up to 25 pounds. Reaching below and above shoulder level and dexterity.
  • Requires eye-hand coordination and manual dexterity to operate office equipment such as computer keyboard, photocopier, telephone, and calculator. 
  • Works closely with other members of the FHH team. Has access to confidential information.
  • Due to the nature of certain assignments within this position, particularly the need to work directly with families in their homes at times outside of the normal hours of operation of the agency, the regularly assigned schedule of hours for this position may deviate from normal office hours. The Family Navigator may need to flex his/her schedule to serve families outside of typical office hours when service deadlines, service effectiveness, and/or family needs make it necessary. 


WORKING CONDITIONS:

  • Work performed in a modern well-equipped environment. 
  • Interfaces with co-workers, physicians, other departments, patients, and outside agencies daily. 
  • Exposure may include noise and biological hazards such as pathogens. 
  • Duties may also be carried out in the community in conditions not controlled by FHH. Operates various office equipment in performance of daily routine duties. 
  • Consistent interaction with patients and the community. 
  • May be asked to lift about 25 pounds.
  • This position could require prolonged periods of sedentary, standing, and/or walking for more than (8) hours per day. 
Salary Description
$17.76-$24.87 + any other compensation offered
