Venture Forthe is one of the largest and fastest-growing Licensed Home Care Agencies in Western NY! We are looking to add a Service Coordinator Supervisor for our Broome, Cortland, Madison, Oneida Otsego, and Delaware counties.  


The Service Coordinator (SC) Supervisor goal is to increase the participant’s independence, productivity, and integration into the community while maintaining the health and welfare of the individual.  The person in this position should be, not only familiar with the participants, but also with SC employees. As part of the SC Leadership team, SC Supervisor will be expected to assist with training, completing annual Service Coordination Supervisor visits, behavioral meetings, completion of 2b forms, as well as other various tasks and projects that may be required of the SC Department. Additional examples of these tasks are outlined later in this document. It is expected that this position model professional and honest communication and address concerns where they are noted. As a part of the Leadership team, this person is expected to work towards a work culture with trust, focus on employee strong points versus weaknesses and communicate appreciation to the employee. Being open to new ideas so the team is able to identify when something can be improved is required for this position to allow us to have an efficient, strong Leadership team.  


This full-time position is expected to hold both ongoing (RSP) and initial (ISP) cases coming on in the region.  Cases will not exceed the manual-determined cap of 25 ongoing cases. This position will be thought of as Supervisor that provides oversight of training of new SC’s when hired. This is a position that is geared towards training, effective oversight of other potential SC staff (sometimes in other regions), as well as other SC department required follow up (Supervision meetings, behavioral/clinical meetings, etc.). Communication related to these job tasks and expectations to the rest of SC Leadership is paramount.  Per manual, SC Leadership can and should assume responsibility for cases in situations where regular SC’s are taking extended time off, paid medical leave, etc. 


 

QUALIFICATIONS:

- An eligible applicant for this position would be required to have a valid NYS Driver’s license with the ability to travel to where their applicants/participants live.

- History working with the TBI and/or NHTD Waiver service population.

- Vetted ability to successfully work independently to submit timely documentation required to complete SC job tasks.

- Vetted ability to successfully manage community resources required to build Service plans and assist with SC training and resources for the team.

-Vetted ability to manage a full caseload, proven ability to understand Waiver rules and ability to educate others on said rules. 


One of the following 3 options must be met: 

  1. Persons self-employed or employed as Service Coordinators must be a/an: 

• Licensed Master Social Worker (Licensed by the NYS Education Department); 

• Licensed Clinical Social Worker (Licensed by the NYS Education Department); 

• Individual with a Doctorate or Master of Social Work; 

• Individual with a Doctorate or Master of Psychology;

• Individual with a Master of Gerontology; 

• Physical Therapist (Licensed by the NYS Education Department);

• Registered Professional Nurse (Licensed by the NYS Education Department); 

• Certified Teacher of Students with Disabilities (Certified by the NYS Education Department); 

• Certified Rehabilitation Counselor (Certified by the Commission of Rehabilitation Counselor Certification); 

• Licensed Speech Pathologist (Licensed by the NYS Education Department); OR 

• Occupational Therapist (Licensed by the NYS Education Department). 

An individual meeting any of the qualifications above must also have, at a minimum, one (1) year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors. 

  1. Persons self-employed or employed as Service Coordinators who have a Bachelor’s degree must have three (3) years of experience providing case management/service coordination, information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors. 
  2. Persons self-employed or employed as Service Coordinators who have an Associate’s degree must have five (5) years’ experience providing case management/service coordination, information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors. 

All agencies that employ two (2) or more Service Coordinators must provide supervision by an individual who fully meets the qualifications as a Service Coordinator. Service Coordination supervisors are responsible for providing ongoing supervision and training to staff.

Requirements

 The Service Coordinator Supervisor is responsible for knowing and following all Service Coordination job duties described in the current manual for each program. Service Coordinator Supervisor are also responsible for tasks identified in these manuals related to Supervisory tasks, including but not limited to: 

  1. The SC Supervisor position must be knowledgeable and able to train others on waiver rules and regulations related to Service Coordination. The Service Coordination Supervisor must have complete understanding of expectations from other departments SC works closely with (Nursing, Compliance, QA, Scheduling, etc), in order to educate and understand how to problem-solve effectively. 
  2. Meet any potential participants face-to-face prior to the completion of the ISP developed by a Service Coordinator, unless doing so would delay completion of the ISP. SC Supervisor documents this visit to ensure annual upkeep. 
  3. Annual SC Supervisor meetings are required by manual. SC Sup may meet any waiver participants face-to-face prior to the completion of the RSP developed by a Service Coordinator under their supervision. Documentation for this visit is required. 
  4. This position will conduct Supervision of SC staff on an ongoing basis with review of caseload and documentation needed to meet waiver regulations. Have supervisory meetings with staff on at least a monthly basis (Venture For the compliance requests meetings biweekly) and maintain notes on these meetings.
  5. Review and sign-off on any/ all service plans for submission to the Regional Center. 
  6. Document progress of goals and problematic performance of SC staff to effectively evaluate staff on an annual basis via performance evaluations. 
  7. Team building, education and positive workplace interactions are required for Supervisors to maintain relationships with SC staff and other VFI Departments. It is expected that this role is always professional and open to discussion and able to navigate problematic situations independently.

Exerting up to 20 pounds of force occasionally and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.


WORK ENVIRONMENT:  Office and community based for first 90 days, thereafter, community based and potential for remote work.
