JOB SUMMARY
Community Health Workers (CHWs) are the backbone of community health at Ellenville Regional Hospital. These vital members bridge the gap between residents and healthcare, ensuring everyone has access to the resources they need to thrive. They act as health education champions, providing culturally sensitive information on disease prevention and healthy living. They also advocate for the community's well-being, identifying and addressing barriers to optimal health. Furthermore, CHWs serve as trusted guides, helping residents navigate the healthcare system and manage chronic conditions. By fulfilling this multifaceted role, Ellenville Regional Hospital's CHW plays a key part in delivering the mission of the Ellenville Regional Rural Health Network (ERRHN). They organize and deliver grant-funded outreach programs outlined in the network's work plans, directly addressing the community's specific health needs.
Specific Elements and Essential Functions
- Conduct outreach to areas of the grant funded catchment area
- Organize and conduct educational events focused grant objectives
- Gather and create educational material on grant focused objectives in both print and visual media to be shared with the community
- Maintain records of grant funded activities for reporting
- Maintains knowledge of emerging best practices on grant funded objectives
- Work closely with ERRHN’s partner organizations, and participate in multi-agency workgroups and collaboration meetings
- Offer social support services to patients of Ellenville Regional Hospital
- Attends project-related meetings and workshops as directed by Executive Director or Program Coordinator
- Participate in community events and health fairs to promote health awareness and Ellenville Regional Hospital programs and services.
- Free Training will be provided to obtain Certified Nutrition Coach (CNC) Certification from The National Academy of Sports Medicine (NASM)

Hours: Full-Time, 36 Hours/Week
Salary: $21.00 – $25.00 per hour based on Education and Experience

EXPERIENCE / EDUCATION
- Associates or Bachelor’s degree in Health or Human Service, Nutrition, Dietetics, Food and Nutrition, or another related field preferred
CERTIFICATIONS/SKILLS
Required:
- Experience working in Nutrition Education
- Experience providing care coordination/management
- Experience organizing projects or efforts of multiple diverse agencies
- Knowledge of social support resources in Ulster County and surrounding counties
- Valid driving license
Preferred:
- Bilingual (English/Spanish)
- Experience working with underserved and rural populations
- Customer service, communication, problem-solving, multi-tasking
- Ability to communicate effectively, both orally and in writing.
- Proficient in computer software programs such as Microsoft Office
