Job Overview:


The PACE Intake Coordinator is responsible for developing and implementing a strategic plan to increase program enrollment for PACE. Leads the enrollment initiatives and ensures enrollment process is efficient and easily accessible for potential enrollees.


Position Responsibilities:

  • Evaluates potential new enrollees, determines levels of care, communicates with the appropriate team members.  Assessments are conducted in the home of the potential enrollee.  Coordinates level of care determinations with the MO HealthNet Division personnel.
  • Responsible for collaboration with inpatient Case Management staff, outpatient departments, physicians, physician liaisons, community referral sources and PACE staff in assessing, identifying and navigating the intake process for potential participants.  Builds and maintains strong relationships and referral patterns among key medical professionals.  Develops and maintains extensive knowledge of inpatient and outpatient programs and procedures.
  • Responsible for the development and implementation of a strategic plan to increase program enrollment for PACE.
  • Works closely with marketing staff and community events to ensure proper audience is being targeted and appropriate information is relayed about the PACE program.  Coordinates educational events for potential referral sources and community service groups.
  • Maintains required statistical data to identify and track issues, trends and opportunities.
  • Maintains current up to date knowledge of Medicare/Medicaid and be able to communicate this with professionals and potential enrollees.  Assists potential enrollees with the Medicaid application process and follows the process to completion.

  • Motivate and lead various participants at all levels in the organization.
  • Effectively use computer to process appropriate data and information and prepare reports.
  • Effectively utilize computer and appropriate software and interact as needed with Jordan Valley Senior Care Information Systems..

  • Physical demands: While performing duties of job, employee is required to move about the Jordan Valley Senior Care center and actively engage with the interdisciplinary team and participants. Employee must be able to sit for extended periods of time, as well as work at the computer for long periods. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for phone communications.
  • Work Environment:  Busy Adult Day Health Center (ADHC) and clinic environment serving the frail elderly with medium to loud noise levels and controlled office temperatures including offsite marketing and home visits for Level of Care Assessments.

Requirements

Experience & Education:

  • Bachelor's Degree, LPN or RN Licensure
  • 3 years with one year with the frail or elderly population
  • BLS Certification

Knowledge of:

  • Techniques of project management, quality, and research methods.
  • Culture and needs of the socially and ethnically diverse population Jordan Valley Senior Care serves.
  • Principles and practices of health care, health care systems, and managed care.
  • Medicare and Medicaid benefits and regulations.
