Job Summary and Deliverables
The Therapist provides care coordination, treatment planning, individual psychotherapy, and group psychotherapy using evidence-based treatment approaches. The Therapist collaborates with the treatment team and community partners to help clients step down from higher levels of care.
- Creates individualized care plans with detailed goals and objectives
- Provides individual and group evidence-based psychotherapy interventions
- Utilizes evidence-based interventions and tools to assist with treatment, i.e. drug testing, etc.
- Provides targeted case management services as described by the Medicaid manual as necessary
- Collaborates on client care priorities with Valley’s multidisciplinary teams
- Collaborates with community partners to meet client and programming objectives
- Assists with program support needs as necessary
- Completes documentation within required timeframes
- Meets client care hour expectations
- Master’s degree - Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or related field
- ACMHC, AMFT, CSW licensure in the State of Utah
- CMHC, LMFT, LCSW licensure in the State of Utah
- Valley de-escalation certification (provided)
