Gay Men's Health Crisis, Inc. (GMHC) seeks a Financial Management/SURE Case Manager responsible for the delivery of case management services to Financial Management/SURE clients.
Essential Job Functions
The following duties are mandatory requirements of the job:
- Coordinate retrieval of documentation needed for enrollment process
- Maintain regular follow up contact with clients HRA case managers/workers
- Refer clients to other GMHC services
- Follow up requests with HRA case workers for client current (RPR) Rent Portion Responsibility and copy client current lease
- Review and follow up on monthly utility bills on behalf of clients
- Maintain all relevant and up to date client data in charts/electronic records
- Comply with state regulations and applicable documentation including statistic reporting
- Complete all required SSA benefit/accounting forms
- Case management
- Mail clients documentation needed
- Perform all other related duties as assigned by supervisor
Education and Certification
- Bachelor’s degree preferred, or two years college level courses in human services related field or a combination of education and experience
- At least 1 year of experience in working with the LGBT population is preferred

- Interpersonal capacity for empathic understanding and nonjudgmental attitude when engaging clients with substance use problems
- Excellent written, verbal and interpersonal communication skills
- Demonstrated ability to work effectively as part of an interdisciplinary team is essential
- Must be a professional and responsible person who is able to develop and maintain interagency liaisons as a means of meeting clients' needs
- Excellent time management skills
- Adaptable to changing priorities and deadlines in a fast-paced work environment
- Intermediate knowledge of Microsoft Office Suite; Excel, Outlook, etc.
- Bilingual Spanish/English helpful
