Schedule: Mon-Thurs 8:30am-5:00pm Fri 8:30am-3:30pm

About the role:
Are you passionate about supporting Veterans and their families? We are seeking a dedicated OMAP Case Manager to provide veteran centered case management services to homeless veterans and veterans at risk of homelessness. This role assists the veterans in, coordinating the provision of VA benefits and other public benefits, short term financial assistance, and supportive services needed to enable clients to obtain and/or maintain housing stability.
Key Responsibilities
- Receive referrals, screen applicants, and determine eligibility under SSVF guidelines.
- Conduct comprehensive assessments and work collaboratively with Veterans to create individualized housing stability plans.
- Provide ongoing case management, advocacy, and crisis support.
- Coordinate VA benefits, public assistance, and community-based services.
- Facilitate temporary financial assistance, including rental and utility assistance, moving costs, transportation, childcare, and emergency needs.
- Maintain accurate documentation and enter client data into HMIS.
- Manage an active caseload of approximately 25 Veterans/families.
- Build strong relationships with VA partners, veteran organizations, and community agencies.
Qualifications
- Bachelor’s degree preferred.
- Veteran applicants strongly encouraged to apply.
- Minimum of 2 years’ experience working with Veterans and/or very low-income individuals or families.
- Familiarity with VA systems, public benefits, and community resources.
- Strong organizational, communication, and documentation skills.
- Ability to work independently and as part of a collaborative team.
- Commitment to confidentiality, cultural humility, and client-centered care.
Physical & Job Requirements
- Valid driver’s license and reliable transportation required.
- Ability to lift up to 30 lbs.
- Comfort working in office and field settings, including occasional outdoor work.
- Ability to manage crisis situations and occasional irregular hours.
