Case Manager
Description


The Case Manager works with diverse populations with multifaceted needs. Primary responsibilities include, but are not limited to, assisting clients in following through on established treatment plans, enhancing social functioning, and improving the client's quality of life. 


Essential Functions: 


1. Verifies health insurance coverage on admission.

2. Conducts a case management intake for patients in assigned caseload, forming a preliminary assessment of substance use disorder and mental health issues. 

3. Coordinates with interdisciplinary team to share information regarding client's status; provides problem solving with challenging patient situations.

4. Provides advocacy, referral, and linkage to community resources and support networks to address identified needs. 

5. Forms and updates treatment and discharge plans in collaboration with the interdisciplinary team and patient. 

6. Obtains entitlements and/or benefits for which the patient may be eligible. 

7. Refers patients, when necessary, to psychiatric services, recovery programs, mental health community support, social service agencies, and/or nursing homes. 

8. Other duties as assigned.

Requirements


Education: 

  • Bachelor's degree required. 


Experience: 

  • Minimum two years of experience working with homeless services programs or provision of social services. 
Salary Description
$55,000 per year
