What You’ll Do
- Deliver compassionate, hands-on care navigation and skilled nursing for clients in their homes
- Help older adults stay safe, healthy, and comfortable at home — avoiding unnecessary hospitalizations
- Manage medications, coordinate care, and communicate closely with families
- Perform skilled nursing services such as IVs, wound care, blood draws, and catheter care
- Partner with leadership and medical teams to ensure exceptional client outcomes

What You’ll Bring
Active RN license (Required)
BLS and First Aid Certification
A caring heart, professional attitude, and passion for patient-centered care
Excellent communication and problem-solving skills
Experience with older adults or care coordination a plus
Other Requirements
Valid driver’s license & auto insurance
Current CPR, annual TB test, and flu vaccine
Clear background & drug screening
