Description
Responsibilities
- Develops individualized treatment and discharge plans for residents
- Makes referrals, coordinates services, and maintains regular and on-going consultations with other organizations
- Coordinates the vocational and educational goals of residents
- Facilities group and individual counseling sessions
- Supports and encourages resident participation in community self-help groups
- Assists in the oversight of program medication procedures
- Conducts breathalyzer and toxicology screenings
- Facilitates new resident assessments, admissions, and orientations
- Availabe for on-call coverage when needed
- Completes documentation per agency requirements
- Data input and word processing
Requirements
- Masters Degree in Human Services or related field and a minimum of 6 months counseling experience. Field placement as an intern may be applied to the required counseling experience. Must possess and maintain a current Qualified Health Professional status. Or
- Bachelors Degree in Human Services or a related field and a minimum of one year counseling experience. Field placement as an intern may be applied to required counseling experience. Must possess and maintain a current Qualified Health Professional status. Or
- Associates Degree in Human Services or a related field; must possess and maintain a current Qualified Health Professional status and two years counseling experience in the appropriate field. Field placement as an intern may be applied to required counseling experience.
- Able to ensure confidentiality
- Must posses a New York State driver's license with an acceptable driving record & reliable automobile

$60,000/Year
