PURPOSE:
The purpose of this job is to assist individuals with disabilities access the community and volunteer sites. To build relationships throughout the community that are receptive to involving those with a disability. To keep the daily records on clients and their daily activities and progress. To implement and oversee the daily scheduling of person-centered program activities. Independence and professionalism is a must in this position. Knowledge and understanding of Autism and other developmental disabilities is key to the success of the client.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provide observation, instruction, and support to clients being served under Community Access Group services, including but not limited to organization, and planning.
- Create a comprehensive schedule emphasizing purposeful community inclusion and meaningful learning activities
- Facilitate and supervise small group community outings and volunteer sites
- Complete goal tracking, attendance, communication logs, progress notes, and other needed documentation.
- Assist with development of natural supports in the community.
- Provide vocational training to individuals interested in employment
- Work with community members to facilitate in-house, hands on workshops
- Assist individuals to maintain health and safety and activities of daily living.
- Provide transportation in all agency vehicles or personal vehicle.
- Attend and participate in Team and Division Meetings, ISP’s, HRST’s, SIS, and other training opportunities
- Participate in all cross trainings as required by the agency.
- Communicate with co workers and other staff regularly on client progress, issues and concerns as they arise
- Other Duties may be assigned.

EDUCATION and/or EXPERIENCE:
- High school diploma required.
- Experience working with individuals with developmental disabilities is preferred.
- Must have reliable vehicle, good driving record, and auto insurance.
- Must have a Georgia State Driver’s License, Georgia registration and a privately insured vehicle
- General knowledge of person centered philosophy.
