Position Summary: The Refugee Resettlement Health Coordinator serves as the primary contact for the JFS Resettlement team working alongside all resettlement clientele in navigating all necessary healthcare services. This position is responsible for scheduling and coordinating initial health screenings for all clients, including medical referral submission. Provides additional assistance and coordination of post-screening appointments for those with serious medical conditions. Assists clientele with Medicaid management, including helping pay bills, maintaining eligibility, and informing clientele of medical insurance plans. Oversees health orientation to newly ORR-eligible clientele. Organizes all logistics to arrange transportation for refugees to attend medical screenings and follow-up medical appointments. The position requires extensive direct work with clientele as well as coordination with numerous local clinics, agencies, and non-profit organizations. 


Salary $27.00 per hour


Colorado residency is required.

Responsibilities:

  • Ensure, the requirement of assigned newly arriving refugees receiving health screenings within 90 days of arrival is achieved.
  • Ensure that newly assigned walk-in cases receive all recommended health screenings and immunizations.
  • Coordinate referrals to medical partners for assigned cases using the REDCap database and other methods as needed (within 7 calendar days after arrival or upon Reach enrollment for non-newly arrived cases).
  • Ensure transportation is provided to and from all initial required health screenings for assigned cases, including but not limited to, all Class A or B initial follow-up visits, Class B TB initial appointments, 2 health screenings required for all newly arrived refugees, and initial immunization appointments.
  • Assist assigned cases with medical visits and specialist referrals when appropriate, including ensuring the provision of transportation to and from the medical providers when needed and emergency services are not required.
  • Work alongside case managers for pre-arrival preparation for clients with listed medical needs and/or cases with high severe medical needs.
  • Seek to eliminate all missed and late medical appointments. Meet the missed appointment goal and other outcomes set by CRSP in collaboration with other Resettlement staff members and the clinics.
  • Provide an initial health orientation to all assigned cases.
  • Assist assigned community members with establishing a medical home/primary care of their choice within 6 months of the screening process.
  • Ensure that assigned families are enrolled in WIC if they are eligible.
  • Assist with continuity of medical coverage through private, public, and charitable medical services.
  • Serve as a liaison between community members and medical providers when necessary.
  • Assist with the resolution of outstanding medical bills as necessary.
  • Network with community resources and clinics to maintain relationships, expand medical coverage opportunities, and supplement services available.
  • Document services in JFS’s database.
  • Add documentation to a client’s case file, including screening referrals, overseas medical documents, and if available, immunization and health screening records.
  • Attend all appropriate network meetings.
  • When requested, assist the Care Team with housing setups for new arrivals.
  • This position will have supervision responsibilities for volunteers.
  • Must be open to other duties as assigned. 

Work Environment:

  • Work is primarily in the office but can include a large amount of travel around the community.
  • Mileage is reimbursed.
  • Hours outside of regular operating hours are required on occasion.

Requirements

 Qualifications: 
Education:

  • Bachelor’s degree (B.A.) or equivalent combination of education and demonstrated experience.

Experience

  • Knowledge of the public benefits in Colorado such as Medicaid and WIC, is preferred. 
  • Language capacity in common refugee language(s), is preferred. 
  • Self-directed, motivated, and able to handle multiple projects simultaneously.
  • Excellent written and verbal communication skills.
  • Negotiation and conflict management skills.
  • General knowledge of refugee resettlement practices in the U.S. is preferred.
  • Experience working in a nonprofit or social services environment is preferred.
  • Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
  • Proficient with or able to quickly learn various computer and web applications.
  • Must have a driver’s license, a good driving record, and access to own transportation.
  • Commitment to the mission, vision, and values of JFS.
